Interventions to Promote an Environment of Safety
As a nurse, we implement plans to promote safety within our patient's environments. The plan
of care for Mr. Smith is to discuss the importance of an environment of safety so he can be discharged to
his home. The plan of care is to promote the importance of;
1. Educating how to check heart rate and blood pressure correctly.
2. Accurately take vital signs regularly, including blood pressure, heart rate, and respiration.
3. Organization of medication, which includes following a schedule.
4. Exercises to improve strength in the right extremities, prevent falls, DVT, and skin breakdown.
Doing these interventions will not only ensure Mr. Smith will have a safe environment when he returns
home but that he will have a healthy recovery at home as well.
The first and possibly the most critical intervention is to educate Mr. Smith on how to take manual
blood pressure correctly and how to check his heart rate. If vital signs are wrong, false reading will be
reported. If Mr. Smith doesn't correctly take his blood pressure, he cannot monitor for any changes,
leading to falls or other sentinel events. Mr. Smith will need to report any changes in his blood pressure,
pulse, and respiration to his provider immediately.
The third intervention the nurse will promote will be to organize Mr. Smith's new medication. The
nurse will suggest Mr. Smith get a weekly pill counter for morning and night time; this way, Mr. Smith can
easily see which pill to take and when. This is due to Mr. Smith being prescribed his new medication,
Coumadin, for his new diagnosis of AFIB. We don't want Mr. Smith to get his Coumadin medication
mixed up with his already prescribed Metoprolol for his blood pressure. Having this plan set in place will
allow Mr. Smith to have an easy way of keeping track of his new medications and reduce the risk of
overdosing due to confusion from his recent stroke.
The last nursing intervention I would like to promote to the patient is how important it is for
exercises to improve Mr. Smith's right-sided weaknesses. Since Mr. Smith sufered a cerebrovascular

accident, he has developed right-sided weakness in both his arm and leg. Mr. Smith also uses a walker to
move efectively around. With his leg and arm in their current condition, he will not be able to ambulate.
Impaired physical mobility can lead to unforgivable severe complications such as falls, deep vein
thrombosis (DVT), and skin breakdown. Mr. Smith or his caregiver will need to plan to assess his skin
integrity daily, ensuring there is no skin breakdown present. According to Fernandes et al. (2015),
"recent evidence has indicated that strengthening exercises increase strength, improve activity, and do
not increase spasticity." Even 30 minutes of occupational therapy daily can lower Mr. Smith's risk of
readmission due to a fall, says Kumar, an adjunct assistant professor at Brown's School of Public Health
(2019).
