Subjective The patient is a male college student who comes to the
facility in need of Dexedrine for his ADHD.
Greg is a college student who according to his statement, has
been suffering from ADHD since childhood. His self-
reported ADHD symptoms include poor concentration, poor
focusing, getting distracted, forgetting things, and feeling
hyper all the time.
The patient reveals that he stopped taking Dexedrine and this
has worsened his ADHD condition. The above-mentioned
symptoms have taken a toll on his grades at school.
The patient comments that he urgently needs to resume taking

Dexedrine in order to improve his condition and improve his
grades at school.
However, there is no past medical history since the patient
cannot recall his previous physician nor clinic. This is largely
due to forgetfulness after he stopped taking Dexedrine one
year ago.
However, there is need for addition information such as his
substance abuse history, possibility for ongoing or previous
supplements intake, his routine for caffeine intake among
others.
Objective The patient has been taking Dexedrine since his early ages
until one year ago when he stopped taking the drug.
Since there are no lab tests for diagnosing ADHD, below are
some of the approaches that should be employed to diagnose
the condition; these include interviews with the patient, close
monitoring of the patient, questionnaire or rating scales.
Also, the physical must conduct Mental State Examination
(MSE) must be undertaken to describe the patient’s current
state of mind particularly under the domains of risk, speech,
thought process, mood, attitude, cognition, affect, and
thought form and content.

persisted for more than 6 months. He has been under
medications since the age of 5 years. Noteworthy, the
patient’s symptoms are negatively affecting his academic
activities.
ii. The patient’s ADHD symptoms (inattentive or
hyperactive/impulsive) were present before the age of 12
years.
iii. The patient’s symptoms are present in more than two
settings (ADD Resource Center, 2019). These symptoms
are adversely affecting his academic performance and
relationships with his parents (mother).
iv. The patient’s dropping academic performance is a clear
