EXECUTIVE SUMMARY
The following report is a proposal for a Non-profit organization that addresses problems in
African American Health care. The following report provides clarification and analysis of the needs
and value of the Mahogany Health Project program implemented in low income communities.
There is an ongoing issue of the health care disparities of the underserved population, especially in
the African American community. Access to healthcare as always been an issue with the underserve
population. African Americans has the highest percentage of chronic illness, and yet they have the
hardest time accessing quality health care in America. Furthermore, many African Americans in
low-income communities lack the basic health education and healthcare management. The
Mahogany Health Project sees an opportunity to help the underserve community in bringing
awareness to heath care disparities among African Americans. Also help bring together a
community to better their lifestyle and health choices through education and health development.
Implementation of the Program will provide health services to the community, as should feel
the need to utilize them to help them achieve a healthier lifestyle. In the Future we hope to gain
national attention to health care disparities and be a national program and foster a systematic
change in the health care system for African American living in low-income communities, and all
African Americans. Our goal is to goal is to help reduce the number of chronic illness, preventable
diseases and help educate the African American Community in Health Education.
The Mahogany Health project will target low-income African American health and health
education. Information presented will tackle the challenges that the community face on daily basis.
It will also on the needs of the African American community and what they want to see changes in.
We will focus on an array of issues and will find better solutions to the problems African Americans
living in low income community face. The program community members and local churches will
be heavily involved in support of the program and adding significant benefits dependent upon their

role in the community.
The Mahogany Health Project marketing plan focuses on bringing together community partners
and resources in a collaborative and effective way to help support the organization’s goal. Services
provided will include outreach health education, support groups and healthcare management.
Our Program management and staff will be qualified and cable of all administrative roles to
ensure that the program is meeting the needs of the community. The program team will consist of a
wide range of healthcare professionals including registers nurses, physicians, health educators and
community outreach personnel. The program will also consist of a staff that will provide strategic
planning and advocate funding for the program. The Board of directors will serve in implementing
and evaluation of the program. The Board of Directors will include representatives from
community organizations and professionals from various fields.
The program will be funding by government grants through a 501 (c) 3. Furthermore, the program
will seek donations through fundraising and from local support in the community. These funds will
help our project with all health materials and research pertaining to bettering African American
Health. The Program will be Fundraising so that it can provide us with supplemental funds that
enhance what an our organization does and provide new opportunities for members. It can also
benefit the community or an individual in the community that needs financial assistance.

Vision
African American health is the mind body and spirit. Taking responsibility in one’s health and
helping the community achieve great health
Mission
Racial minority groups in the United States are at a higher risk of being uninsured, lacking

access to health care and experience worse health outcomes form preventable conditions (1). Poor
mental and physical health outcomes, delay or failure in seeking preventive services and treatment,
poor adherence behaviors have been all associated with reported experiences of discrimination
among racial/ethnic minority (2). High mortality rates amongst the African Americans population
especially low-income is frequently reported due to cost of health care services. A non-profit
organization I created is the Mahogany Health Project which helps reduce the number of health
cases in African American Communities by providing health education, health services and much
more. Furthermore address health disparities amongst the African American population.
The Mahogany health project is committed to improving the health and well-being of African
American who live in low-income communities by providing disease prevention and disease
management programs, health and educational services and events.
Core Values
Church and doing God’s work is the background of our organization. We believe that Spirituality
continues to be the core value within the African American Community for citizen engagement,
leadership development and self-sufficiency.
Goals
The Mahogany Project’s goal is to help reduce the number of chronic illness, preventable diseases
and help educate the African American Community in Health Education.
Performance Objectives
By 2023, Build a culture of preparedness in the community and encourage People to have a
plan of action regarding their health
By 2025, the program planners for my program will extent the program throughout low-
income communities throughout the United States.
By 2025, Cases of Chronic Diseases in low-income communities of Richmond, Virgina will

be reduced by 21 per 1000.
Durning the program, participants will explain different ways to maintain a healthy lifestyle
within a certain income level.
Section Two
THE ORGANIZATION
Andrea Woodley Executive Director Andrea Woodley is the Executive Director of the
Mahogany Health Project Program. Andrea brings over 5 years of experience in the
corporate and nonprofit sectors, particularly in areas such as strategy development and
analysis, program management, and working with multicultural teams.
History - Established in 2016, The Mahogany Health Project Program was developed out of
the Richmond VA Black Chruch Initiative. This was in response to an increasingly urgent
need to decipher and negotiate the diverse health care needs of minority communities the
growing refugee and immigrant populations of the US. From 2012-2016, The Black Church
Initiative has provided help to local churches in the inner-city area of Richmond Va.
Creating programs to help serve the less fortunate, clothes drive, tutoring sessions for
children and adults and Job fair for ex-offenders. In 2017, the Richmond VA Black Chruch
Initiative made the decision to focus its work solely on the health disparities facing African
Americans and officially changed its name to The Mahogany Health Project.
Mahogany health project is improving the Lives of African American who live in low-
income communities. Our Program provides disease prevention and disease management
programs, health and educational services and events. Some activities include Obesity
prevention activities, Free STI/HIV testing, Prediabetes screenings, Mammogram
screenings, Health Education Classes for adolescent youth, Physical Activities for the

elderly and Mental Health programs

We celebrate 3 years of innovation and progress, we continue to expand our services and
our concept of ‘health.’ Mahogany Health Project Program is proud to be working with an
increasingly array of health and human service providers and organizations committed to
the health of the communities they serve.
The Mahogany Health Program has completed projects with a variety of partners and clients
over the years.

1. Provide annual breast cancer education, prostate cancer screenings and education, and
several other health screenings
2. Developed a growing network of church liaisons that brings health information to their
congregations and encourages participation in screenings and events.
3. Offer exercise classes, free flu vaccination clinics and free vision screenings.
4. Provide First Aid, Cardio Pulmonary Resuscitation (CPR) and Earthquake Preparedness
training.
5. More than 20 members of the committee and countless volunteers contribute more than

10,000 hours of volunteer time to the Mahogany Health Project activities and programs.

The organization will consist of an array of administrative staff and program managers.

Administrative Staff are those who organize and execute office duties, they interact with the
clients and attended to everyday activities to make sure the program is running how
smoothly.
Health and Human Services are people who help feed the hungry, assist crime and
offenders, provide job training, help house those who are homeless, assist with health care
needs, and assist with programs to help youth mature into adults who contribute to society.

Human Resources Staff are the people that are the point of contact that assist with building
skilled teams to take on community projects. Assist with recruiting and organizing

Volunteers are people who people who work directly with Human resources staffing.
Volunteers help with whatever task within the organization. They may assist with greeting
visitors, guiding programs and help with special events. Volunteers also help with
conducting activities inside and outside the program like classroom and outdoor programs,
assisting with Chruch programs or community programs. (6)

The staff members working with the Mahogany Project will not be provided Health

CORPORATE GOVERNANCE
The Mahogany Project is committed to good corporate governance which will promote the
long-term interest of investors and help the community build trust in our organization. The
Organization will be operated by four Board of Directors. With the Help of both, each member will
help with the oversight of the Organization to make sure legal and financial measures are properly
followed. Furthermore, make sure that the program is running how it should be and ensure the
Mission statement is achieved
Section Five
HUMAN RESOURCE & MARKET ANALYSIS
Racial minority groups in the United States are at a higher risk of being uninsured, lacking
access to health care and experience worse health outcomes form preventable conditions(1). Poor
mental and physical health outcomes, delay or failure in seeking preventive services and treatment,
poor adherence behaviors have been all associated with reported experiences of discrimination
among racial/ethnic minority (2). In addition, the African Americans community has had the worst
health outcomes to treatable conditions such as cardiovascular disease, cancer, and HIV than those
in the majority. African American suffer, from a significant burden of disease from breast cancer,
cervical cancer, and colorectal cancer; causing mortality rates to be higher than those who are non-
Hispanic. Between 2000 and 2010, the prevalence of chronic diseases comorbidities had increased
for Hispanics 32.2%-42.4%, and African Americans 43.8-51.6% over the age of 65.(7) Therefore,
the Mahogany Health Project seeks to make a change in the African American Community.
The marco-environement is that social and economic factors play a key role in health and
health of African American. Studies have shown that African Americans living in urban
communities tend to have the worse health conditions. African American Living in urban

communities is viewed as a burden with enormous healthcare disparities, associated with factors
such as insurance status, income, and educational status.
The Target population for this program is individuals of Richmond City, Virginia age 2-65
years of age living in urban communities. This program will target children from young to old and
educate them on health issues associated with the African American Population. In addition to
providing health services to all ages, such as breast cancer screenings, physical activity for children
overweight or obese, and blood pressure testing.
One Competitor the Mahogany Health project may have, is the Urban League of Greater
Richmond. This Non-profit organization helps to address problems of African American in the
Richmond area. Their Mission is to assist the underserve citizen in archiving social and economic
equality through advocacy, collaboration direct services and research.(8)
Here is a Breakdown of the Mahogany Health Project SWOT Analysis:
Strengths- Our Program will be able to strengthen the African American Community by
creating and offering services and programs addressing Social and Economic Issues of
African American Health.
Weakness- One weakness may possible be a lack of funding for Our Organization. Also
being able to find ways get African American to be a part of our organization and keep them
coming back.
Opportunity- Partnering with other well-known Organization, catering to the same mission
as our program.
Threats- Increase Competition of other Health Organizations for supporters and donations.
Section Six
RESEARCH AND DEVELOPMENT
The Best research method for our program is gathering information from the VA State
health department on communities where African American Health suffers the most and going into
those communities and speaking with the Community Boards and leaders. We send out surveys

throughout the community to help us get a better understand on what Health issues we should
primarily focus on.
Intellectual Property we will use is using the Office of Minority Health programs to help
develop our programs.
One of the most Important activity of the Mahony Health Project is partnering with a larger and
well know organization to help us get stay around for years to come.
Section Seven
STAKEHOLDER RELATIONSHIPS AND ALLIANCES
Stakeholders are individuals or a group of people that has an interest in activity or decision
on an organization (9). Stakeholders are the voice in which helps the organization move in the
direction in which benefits everyone. Engagement from the stakeholder reflects an accepted
attitude that organizations not only have an ethical obligation to involve the participation of
stakeholders in their collective activity but also in so doing their overall organizational
effectiveness will be enhanced (10). Therefore, are several Stakeholders for the Mahogany Health
Project.
The First group of Stakeholder are the community members. This group is important to the
Mahogany project because they will help us identify key health issues of the community and help
us address them. Community members that participate for our organization shows commitment to
help make this project happen. Community members play an important role because without
community members there is no community. In addition, the community's members will help create
more effective solutions to the community problems. Last, community members will be able to
engage in local networking. The more people who know about the Mahogany health project and are
willing to work towards the goal of the organization, the more likely we will be successful in
reaching our goals.

The second group of Stakeholders are the Community health educators and professional. Health
Educators and health professionals will play a valuable role in the our organization. They are the
link to a healthier community, and they will provide a lot of support to our program. Health
educators will help identify health issues and trends within the community. They will work with
community members to find better solutions to the health issues the community face. The
community health workers will work with public health departments, and government offices to
help design educational programs to address the community needs.
The Third group of Stakeholders are the Board of Directors. The Board of directors will help with
decision making. They will be able to introduce new ideas for the organization and help the
organization achieve all goals. They will help monitor they program and provide strategic advice
for our organization. They will also make sure the needs of the community are met and that
community members ideas goes unnoticed.
Section Eight
RISK FACTORS AND REGULATORY COMPLIANCE
The Mahogany Health Project aims to improving the lives of the African American
community. However, with many organizations, we may have a few Risks factors that can threaten
and jeopardize the success of our organization. However, it is our job to find ways to manage and
prevent these risks to our organization. Here are a few risk factors that the Mahogany Health
project may face.
One Risk factor our organization may face is funding. I believe this is the key risk factor
because our organization will depend on donations. Therefore, without funding this can cause a
dominoes effect of the organization, not being able to purchase supplies, advertising and majority
of the work of our organization. How we can overcome this problem is making sure meetings are
held to discuss any issues with our funding. Then create strategies to help the organization with any
funding issues we may have.

Another risk factor is possible Data Breaches. Our organization is main goal is improving
health for African American. We will be education African Americans on the health issue within our
community and we will sponsor Health care programs like free HIV/AIDs and STI testing,
Diabetes testing, blood drives and much more that requires information from the participants. In
addition, the Mahogany health project will be encouraging online donations and electronic
payments. The internet and technology are rapidly growing, and it is important to keep everyone's
information safe. Therefore, our organization will ensure our health care professionals follow
HIPPA guidelines by making sure everyone has the proper training, followed by a form each health
care professional will sign to make sure they are following guidelines. Lastly, we will make sure
our website has the safest online credit card system and credit monitoring system if a data breach is
to occur.
Our Last Risk factor is Tax- exempt status. This can be an issue because the IRS’s approval
of tax-exempt status can be easily revoked. Our Organization is a 501C3 nonprofit organization, if
there are any issues of our organization following the guidelines our status can be revoked. Our
program will ensure that this will not happen, by setting bi-weekly meetings and making sure we
stay within the guidelines of the IRS.
Section Nine
STRATEGIC COMMUNICATION & MARKETING PLAN
Healthcare is described to be the efforts made to maintain or restore physical, mental, or
emotional well-being, especially by trained and licensed professionals. Having access to health care
is significantly important to promote and maintain health, prevent and manage disease, reduce
unnecessary disability and achieving health equity for all Americans. The purpose of my program is
to improve the lives of the African American. Furthermore, tackle health disparities and barriers we

face in low income communities regarding our health
The most common group living below the poverty line in Richmond, Virginia are African
Americans.(11) Studies have shown that racial minority groups such as African American living in
the United States, are at a higher risk of being uninsured, lacking access to health care and
experience worse health outcomes form preventable conditions. (2) In 2013, the state of Virginia
246,000 African American was uninsured. (13) Although now, numbers have decrease, chronic
illnesses amongst African Americans are still an issue. African American suffer, from a significant
burden of disease from breast cancer, cervical cancer, and colorectal cancer; causing mortality rates
to be higher than those who are non-Hispanic. (14)
The Mahogany health project is committed to improving the health and well-being of African
American who live in low-income communities by providing health education classes, health
serveries and health management held by churches located in the Richmond Area. Services will be
available throughout the weekday and weekends for community members to participant in. Each
service and educational class will be free for all adults and children in the community. We will
educate them on chronic illnesses African American face, different health disparities, how to
manage a healthy life on a strict income and disease prevention. We strive to do God’s work and
bring spiritualty and health together for the greater good our community.

understand important health really is. Educational classes and health services will be main
supported through local churches in Richmond. However, we will also provide services
through local community and recreational centers
Value- This program is to help decrease the number of chronic health cases in African
Americans living in low-income communities.
Education- Our will provided free beneficial information to the community. We want to

bridge the gap between educating African Americans on health and the health care system in
the United States.
Marketing Plan
We want to change the lives of as many people as possible, so marketing is very important. The
Mahogany Health Project will be promoted in the community setting. Handing and hanging up
flyers for all of the Richmond communities to see. In the community board meetings, churches,
local radio stations, health departments and in school as well. Social Media will be used as an
additional platform to promote this program. We will use platforms such as Facebook, Instagram,
Youtube and other social media outlets. It is important to portray a positive image for this program.
Something that the target audience can relate to or what they can see their self as. A healthy and
active lifestyle is the image that will be portrayed. We will also collaborate with bigger
organizations and even college campus to help spread the word of our organization.
Section Ten
BUDGET AND FINANCIAL MANAGEMENT
Budgeting for the Mahogany project is a crucial part for our program. As our program is still
underdevelopment, we are currently working on a budget. As an estimated our program is roughly
around 3 million dollars. We want the Mahogany Helth Project to be a long-term program, and this
cost will cover health care expenses, educational program training programs and research. The
Mahogany Health Project estimated to be around a 3-million-dollar project. The duration of this
program will be two years and 6 months. This program will use 2 million the first year and one
million for the second year. The first year of this program will spend most of the money
implementing the cost and creating the program. While the second year of the program will use the
remaining money to keep the program up and running. With the support of the community
members, stakeholders, and health care professionals is expected so much growth after our first
three sessions. Here I provide a breakdown of our budget of our program for the first year;

Totaling out to 2,745,000$. The remaining money will be used for miscellaneous things.
Funding
Funding for the Mahogany health project will be from government grants and local funding.
Government grants are important to have because they are awarded for the purpose of satisfying a
greater need, which helps society(14). The grants for our program will fulfill a purpose and help
the community of Richmond, VA. These funds will help our project with all health materials and
research pertaining to bettering African American Health. These funds granted for our organization,
will be solely used for these purposes and cannot be used for anything else. Local funding will be
consisting of donations and support from the City of Richmond, other communities' small
businesses and large business. Most of the fund would cover marketing and advertising of the
program and needs of the program. Marketing and ADs are important to get the program out.
Marketing will allow the program to be out and gain the attention of the target audience. This
program will need as much exposure to help motivate individuals to make a change in their life and
community. The cost of space is extremely important as throughout the program it will provide
access to health-related activities and educations sessions. From the gym, recreational centers,
parks and local gyms.
Lastly the Mahogany Health program will be also be gaining funds through fundraisers.
Fundraising can provide us with supplemental funds that enhance what an our organization does
and provide new opportunities for members. It can also benefit the community or an individual in

the community that needs financial assistance. (15)
Profit and Projection Loss
While we aim to gain all means of financial support, our organization will not be set to make any
extra money for profits. All money gained will be strictly used for the purpose of our program and
betterment of the community. Any funds that are left over from our budget will be kept to better the
program. To ensure we are in our budget, we will hold monthly meetings regarding financial
matters and any losses we experience.
Investment
In the future the Mahogany project would like to invest and excess money back into the community
and expand our program. Our future goal is to one day have the Mahogany program help all
African American throughout the United States in bettering their health. Furthermore, invest in
health care programs for Low-income communities, more health educational programs, and better
recreational activates for children and adolescents living in poverty or low-income communities.
