The Virginia Department of Education does not require that schools use this sample IEP format; it is
offered as a best practice example. The sample IEP form is divided into two sections. The first section
includes those pages that are the foundation of all IEPs. The second section includes those pages that
will be added to the IEP as needed and sample formats for other purposes.

Factors for IEP Team Considerations: This form may be used to document the team’s
consideration of the matters that the applicable regulations require the team to consider during the

The documentation of these considerations, while not required, is best practice. However, all

The Individualized Education Plan (IEP) that accompanies this document is meant to support the positive process and team
approach. The IEP is a working document that outlines the student’s vision for the future, strengths and needs. The IEP is
not written in isolation. The intent of an IEP is to bring together a team of people who understand and support the student in
order to come to consensus on a plan and an appropriate and effective education for the student. No two teams are alike and
each team will arrive at different answers, ideas and supports and services to address the student’s unique needs. The student
and his/her family members are vital participants, as well as teachers, assistants, specialists, outside service providers, and the
principal. When all team members are present, the valuable information shared supports the development of a rich student
profile and education plan.

During the IEP meeting, the following factors must be considered by the IEP team. Best practice suggests that the IEP team
document that the factors were considered and any decision made relative to each. The factors are addressed in other sections
of the IEP if not documented on this page (for example: see Present Level of Academic Achievement and Functional
Performance).
1. Results of the initial or most recent evaluation of the student;

instruction in Braille or the use of Braille is not appropriate for the student. When considering that Braille is not appropriate
for the child the IEP team may use the Functional Vision and Learning Media Assessment for Students who are Pre-Academic
or Academic and Visually Impaired in Grades K-12 (FVLMA) or similar instrument; and

11. In the case of a student who is deaf or hard of hearing, consider the student’s language and communication needs,
opportunities for direct communications with peers and professional personnel in the student’s language and communication
mode, academic level, and full range of needs, including opportunities for direct instruction in the student’s language and
communication mode. The IEP team may use the Virginia Communication Plan when considering the student's language and
communication needs and supports that may be needed.

The Present Level of Academic Achievement and Functional Performance summarize the results of assessments that identify
the student’s interests, preferences, strengths and areas of need, including assistive technology and/or accessible materials. It
also describes the effect of the student’s disability on his or her involvement and progress in the general education curriculum,
and for preschool children, as appropriate, how the disability affects the student’s participation in appropriate activities. This
includes the student’s performance and achievement in academic areas such as writing, reading, mathematics, science, and
history/social sciences. It also includes the student’s performance in functional areas, such as self-determination, social
competence, communication, behavior and personal management. Test scores, if included, should be self-explanatory or an
explanation should be included, and the Present Level of Academic Achievement and Functional Performance should be
written in objective measurable terms, to the extent possible. There should be a direct relationship among the desired goals,
the Present Level of Academic Achievement and Functional Performance, and all other components of the IEP.

Mary is a 3-year-old preschooler. Her primary exceptionality is developmental delay. She is
diagnosed with Trisomy 21 (Down’s Syndrome). She also has asthma and hydrocephaly. Mary has been
hospitalized twice due to respiratory complications. She takes medications (Flovent and Albuterol) for
her asthma condition, as well as an allergy medication (Zyrtec) and a reflux medication (Prevacid).
Mary drinks only thickened liquids and has a lactose intolerance. She has no food allergies. She has been
seen for a swallow study and she will likely be seen by an ENT in the future. She will also have a GTI
scope and be seen by a pulmonologist. She is immunocompromised.
Mary demonstrates strengths in responding to social interactions. She is responsive to familiar
adults and open to others’ initiation of interactions. She is responsive when her name is called. She
displays pride in her accomplishments and will vocalize to express her feelings. She is able to move
independently around the house, to walk up and down stairs with support to maintain balances when
going from standing to sitting or other non-vertical positions. Additionally, she also demonstrates some
hand coordination. She can feed herself bite-sized piece and drink from a cup with assistance. She can
manipulate objects, drop and search for objects, and extend and release and throw objects. Mary also
plays with shape sorters and puzzles.
Mary is unable to imitate circular drawings on a page, string beads or stack cups. She does not
demonstrate attentiveness to teacher directed activities for more than 3 minutes. Mary is also unable to
kick a ball without losing balance. She also demonstrates lack of awareness of common dangers and will
put non-edible items in her mouth. She is also not yet able to match objects by their properties (color,
shape, size, etc.) or to identify common use of objects. She also does not refer to herself by pronouns nor
name.
Mary’s teacher reports that she is not yet following the routines of the classroom, neither is she
sharing with classmates nor taking turns with them. Mary does not yet participate in group activities.
Mary is also an elopement risk. She will wander away if not continuously and closely watched by an
adult.

Mary’s mother also reports that Mary displays the same tendencies to wander away at home as
well. Mom additionally reports that Mary will make a mess with foods. She does not eat much meat or
raw vegetables and prefers pasta. Mary also does not like clutter and will scatter her toys if there is too
much in front of her. Mary’s mom also reports that she has received a Level III AU diagnosis recently.
The teacher reports that Mary will become fussy when in an unfamiliar situation or one that
makes her feel nervous. She looks for adult support and prefers benign held when distressed. Mother
reports fussiness at home as well but only in response to being hungry, thirsty, or tired. The difference in
fussiness antecedents seems likely due to the difference in settings—there is less likely to be anything
unfamiliar or nervousness-inducing at home.

services written in an Individual Education Program (IEP), the federal government can help the public school
