Mental Status Exam
Mr. McGuire attended initial interview dressed business casual and weather
appropriate. Mr. McGuire was wearing khaki pants with a navy-blue button up shirt with short
sleeves. Mr. McGuire wore a white t-shirt underneath his dress shirt and brown shoes. There
were no signs of holes or tear in his clothing. His hair was parted to the left side and well kept.
There were no visible tattoos, and he kept a good posture. There was no presence of body odor or
wounds. He was groomed and neat. He appeared engaging, polite and pleasant. No signs of
current drug use were displayed.
Due to being nervous Mr. McGuire demonstrated difficulty being still
throughout the interview by shaking his left leg up and down occasionally or rubbing his fingers
together. He appeared tense and nervous. Mr. McGuire shared that he occasionally plays the
drums in his home church which clarifies the shaking of the leg and the fixated posture. Mr.
McGuire was polite and introduced himself at the beginning of our interview. He was friendly
and smiled and he introduced himself. Mr. McGuire was attentive in his body language by
leaning a little forward at times and making eye contact. He also utilized verbal encouragers and
some humor to feel more at ease during our conversation. There was no awkwardness or silence.
Overall, psychomotor activity was within normal limits and his speech was at a normal rate. No
force speech or slowness presented during the conversation. He did utilize rapid speech at the
beginning of the interview due to his nervousness, but he adjusted his speech to a normal rate
after getting more comfortable.
Mr. McGuire was very concentrated and alert. He was able to answer all
questions promptly and accurately according to the topic or question being discussed. He appears
to have a memory above average as evidenced by his ability to recall even small details from

past events. He presented no disturbances in his thought process. He adequately discussed his
aspiration of being part of the mission’s trip and working in a difficult environment. He
demonstrated awareness of himself and his surroundings. Additionally, Mr. McGuire was not
easily distracted and possess good social judgment. He also appeared to have intelligence above
average and consistent with his level of formal education. This as evidenced by his lexicon and
honorary awards in college.
Mr. McGuire demonstrated to be coherent and logical in thought process and
logical. There were no evidence of flight ideas or looseness of associations. He continued to
express his passion for helping others and for his faith. Mr. McGuire was not compulsive or
obsessive in expressing his ideas. He did not demonstrate to have any hallucinations or delusions
or any apparent ideation.
Sensory motor and perceptual process were within normal limits. Mr. McGuire
presented no evidence or fine motor tremor, auditory, or perceptual impediments. He
demonstrated ability to analyze information, detect patterns in the questions being asked and
connections between different pieces of information being presented.
Through the initial interview Mr. McGuire did not demonstrate or indicate the
presence of any negative mood. Mr. McGuire demonstrated appropriate emotions according to
the situation by expressing excitement and smiling. He did express being nervous at the
beginning of the interview which was congruent with first meeting and demonstrated by his
tenseness at points during the interview. He also did not demonstrate any impulsivity, any violent
behaviors, or thoughts, and no homicidal or suicidal ideation. Mr. McGuire demonstrated
appropriate impulse control which are consistent with his history. He appears to be well-adjusted.
