Reason for testing/Referral Question:
My client, Miss. Daisy Cambillo – Ortega was referred initially from a local Christian
church in Charlotte, North Carolina. This referral evaluates the examinee's ability to be a good
Resident Assistant (RA) at Liberty University. The client is conscious that the RA position can
require participation in staff meetings requiring critical decision-making; another key
responsibility is mediating conflict among residents and enforcing regulations and policy. Due to
the role's high responsibility, the primary purpose of this referral is to ensure that Miss. Daisy
Cambillo-Ortega is mentally capable of working in a fast pace position and making important
critical decisions.

Examinee Background:
Daisy Cambillo- Ortega is a thirty-two-year-old Hispanic female who grew up in a
predominately catholic household. Miss Cambillo-Ortega is single and has no children. Miss
Cambillo-Ortega is an active disciplined individual. Her daily life consists of an early start, a
nutritional breakfast, and walks with her Pomeranian. Miss. Cambillo-Ortega’s schedule is
routinely timed strictly due to school. Miss Cambilllo-ortega has a job as a Human Resource
supervisor and mentioned that she had a similar position in a work-study program before. Miss.
Cambillo-Ortega has no medical issues or concerns at this point. Miss. Cambillo-Ortega's speech

was paced normally and seemed talkative with no rapid speech notice. Miss. Cambillo-Ortega
had no sensory-motor and perceptual process delay Miss Cambillo-Ortega appears to have
excellent recent and remote memory. As mentioned, the client's attention did not alarm any
concern. She seemed to be very groomed and sharp in her appearance. Miss. Cambillo-Ortega
presented herself confidently, had excellent eye contact, and was well-mannered. Miss. Cambillo
-Ortega reports a history of psychiatric counseling to help her relationship with her abusive mom
and childhood trauma. She reports going to counseling twice a month. Currently, she wants to
have a holistic approach without any medication usage. According to Miss Cambillo-Ortega, her
mother was diagnosed with bipolar disorder and depression, and she doesn't believe she and her
twin sister have been medically diagnosed with depression or anxiety but does not deny having
some symptoms being away from her twin sister and the changes of school. Miss Cambillo.
-Ortega states that her perceived strengths are compassion, adaptability, empathy, and optimism.
Miss. Cambillo -Ortega reports that her weakness is people pleasing and her inability to say no,
especially when it involves the people she cares about most, and that she aspires to be a License
Mental Health counselor.

subcategories. A low score of 1-10 includes the Individual experiencing minor ups and lows in
mood, which is considered normal. A score of 11-16 is conveyed as a mild individual score of
17-20 is conveyed as a clinically depressed individual on the borderline clinical range. A score of
30 is conveyed as moderate depression, 31-40 is severe, and a high score over 40 conveys
extreme depression. The Client Mrs. Cambillo-Ortega conveyed a score of 10, which is
conveyed as a normal clinical range.
Becks Anxiety Inventory
Similar to the Beck Depression Inventory. It is a test categorized as a 21- items survey.
Suppose a self-report assessment measures an individual's likelihood of having an anxiety-
related disorder. A grand sum between 0-21 indicates very low anxiety, a sum of 22-25 means
moderate anxiety, and a grand exceeding 36 is a concern. Miss Cambillo-Ortega conveyed a
score of 22, indicating mild anxiety.
The IPIP-NEO
The IPIP-NEO assessment estimates a person's level on the five broad.
Personality trait domains, and 30 sub-domains of the Five-Factor Model. It consists of 300 items.
Answers are provided according to a 5-point Likert-type scale. The numerical scores represent a

Conclusion and Recommendation
Based on the results of the assessment, it is the conclusion of this examination that Miss
Cambillo-Oryeha is an acceptable candidate for the position of Ra Assistant for the following
reasons:
a) Miss Cambillo-Ortega possesses personality qualities that will support her ability to adapt
to challenging situations, including the ability to use critical thinking to make complex
decisions
b) Miss Cambillo-Ortega does not suffer from depression, and although the test indicates her

having moderate attendance, it could be normal since she has never been separated from
her twin sister, and those factors can all take time to process
c) Miss Cambillo-Ortega is likely to engage positively with the residents and those who will
work with her
