Individualized Service Plan
(Due on 11/29/18 at 11:15 a.m.)
Objectives:
1.   Students will explore solutions to public child welfare dilemmas including, violence, addiction,
homelessness, and mental illness.
2.   Students will explore interventions available for families whose children are at risk including
differential response, placement, foster care, family preservation, adoption and permanency
planning.
Instructions and Requirements:
1.
  Scenario:
Each student will be provided with a scenario upon which the project should be based.
2.
  Challenges and Strengths (5 points):
Develop a list of 5 challenges facing and 5 strengths
characterizing the client(s).
3.
  Individualized Service Plan (ISP) (4 points):
Write an ISP comprising 1 goal and 3 objectives;
prioritize the objectives. The goal and objectives should be based on the identified challenges
and strengths of the client(s). The written plan
must
follow the required format (see attached).
4.
  Building on Strengths (6 points):
Describe how some identified strengths will facilitate the
client’s achievement of the objectives.
5.   The written ISP should be typed double-spaced and should be 3-4 pages long. The plan must be
well-written and well-organized (

6.   The ISP may be submitted as a hard copy or through the instructor’s email address

7.   The written ISP is due on 11/29/18 at 11:15 a.m. A written plan submitted after 11/29/18 at
11:15 a.m. will automatically lose 4 points. A written plan submitted after 12/6/18 at 11:15 a.m.
will automatically lose 6 points. A written plan submitted after 12/13/18 at 11:15 a.m. will
receive a zero. Neither absence (for any reason) nor technical difficulty is an acceptable excuse
for late submission of the written ISP.
