Summary of strengths:
I'm comfortable doing evaluations, documenting and recognizing the evident indications of
psychological problems, and separating a differential diagnosis depending on the client's
symptoms. I use my knowledge I gain theoretically from class to treat my patients for better
health outcome. Based on my work from the previous semester, I am knowledgeable and
proficient with applying the DSM-5 to obtain a precise diagnosis for a client. I am also
confident in my ability to conduct and interpret a functional assessment for a client. My
experience working as a nurse has enhanced my professional skills like maintaining medical
and therapeutic relationships with my clients and applying evidence based treatment in my
clinical practice.
Opportunities for growth:
I need to improve my knowledge of how to choose the right prescription for a client based on
their symptoms, clinical diagnosis, and practice guidelines, emphasizing the risks/benefits,
side effects, and cost aspects of the medication. I also need to assess the client's response to
drugs and identify whether the medication needs to be increased in dose or switched to a
new prescription or class of meds entirely. I want to improve my ability to use diagnostic
tools and evaluate data, as I haven't done much with them before this semester.
Now, write three to four (3–4) possible goals and objectives for this practicum experience. Ensure that
they follow the SMART Strategy, as described in the Learning Resources.
1. Goal: By the end of the semester, you should be able to determine the right medicine
and dosage based on the client's symptoms and diagnosis and how to change the
medication regimen if it fails to relieve the client's symptoms.
2. a. Goal: Each week, I work with my preceptor to find the most appropriate medication
using the best evidence-based practice.
3. b. Objective: Each week, evaluate the prescribed medications of clients seen in my
clinical rotation to determine the efficacy of drugs prescribed based on follow-up
appointments using Stahl's Prescriber's Guide.
4. c. Objective: Each week, evaluate the prescribed medications of clients seen in my
clinical rotation to determine the efficacy of medicines prescribed based on follow-up
appointments using Stahl's Prescriber's Guide.
5. Goal: By the end of week six of this program, you will be able to use diagnostic
scales/tools to help you determine an acceptable diagnosis for children, adolescents,
and adults.
a. Goal: Work with my preceptor each week on the techniques he utilizes to help him
determine an appropriate diagnosis for the people he sees during my clinical rotation.
b. Goal: Once a week, I will research the most effective diagnostic tools/scales utilized
in clinical practice to determine the adult client's diagnosis.
c. Goal: During this course, I will review one day per week the tools/scales that are
specifically intended for children and adolescents until I am confident in using them in
clinical practice.
6. Goal: By completing this course, you will be able to establish the most appropriate
diagnosis and treatment plan for a child who appears for evaluation based on the
child's presenting signs and symptoms.
a. Goal: collaborate with my preceptor to establish the most suitable assessment
tools to utilize when determining the perfect diagnosis for my clients

week.
c. Goal: Review childhood mental challenges every week by use of approved evidence
based methods.
7. Goal: By week 11 of this course, you will have determined the most appropriate
treatment plan based on the client's assessment who presents with psychological
difficulties.
a. Goal: Work with my preceptor each week to assess whether a client will benefit
from psychotherapy alone or if the medicine should be given to help them overcome
their issues.
