PREVALENCE AND MODES OF COMPLEMENTARY AND ALTERNATIVE
MEDICINE USE AMONG PEASANT FARMERS WITH MUSCULOSKELETAL PAIN
IN A RURAL COMMUNITY IN SOUTH-WESTERN NIGERIA.
Abstract:
The present paper focuses on the extent and methods (i.e. CAM) applied by peasant farmers with
musculoskeletal pains from a rural area in the South-Western Nigeria.#The research project
applied mixed methods, including the administration of the questionnaire survey along with the
in-depth interviews.#Volunteer farmers (300) were surveyed using a structured questionnaire and
interviews were conducted covering 30 participants by a team of experts to understand their
experience of CAM.#The survey discovered that most of the peasants were the practice of CAM
and approximately 70% of them were using CAM alternative methods for the management of
musculoskeletal pain.#The most common practices employed CAM included the use of herbs,
massage therapy, and traditional ways of reconstructing broken bones.#The factors affecting
CAM utilization included cultural preferences, accessibility, affordability, and efficacy among
users.#The qualitative context data revealed how the cultural and the socio-economic
environment of the local village was the background of the use of CAMs.#It shows that
understanding of CAM practices in a specific context is crucial and, therefore, the health care
providers need to recognize and integrate traditional treating procedures in every health care
system in order to provide wellness care services to the individuals who are struggling with
musculoskeletal pains in rural areas.
1.0 Introduction.
Musculoskeletal pain is a silent but extensive health problem that affects at least 9.8 million
persons worldwide, manifesting not only as the loss of full function mobility, but also a
depreciation in the quality of life.#These are central to pain that covers a very wide range of
conditions, such as arthritis, either back or joint pain, or injuries to bones, muscles, and
joints.#They are particularly noticeable in the countryside of the LMICs where calling home the
conventional healthcare services is still limited, which consequently leaves individuals no other
choice but to live with their pain and painkillers only.
The instance of musculoskeletal pain within the farm sectors which are characterized by the
major controlling agricultural activity among a majority of the populations of rural Nigeria is a
serious concern.#Often the manually employed suffer under environmentally hard work, such as
doing heavy lifting, repetitive activities with the possibility of risking back injuries and various
other musculoskeletal diseases including arthritis.#Moreover, lack of medical facilities and cares
specialists which are prevalent in rural areas also contribute to the pain experienced by people
that are looking for relief form musculoskeletal issues.
While the coverage of conventional health care system may pose a barrier to those in the rural
area of Nigeria, the popularity of complementary and alternative medicine (CAM) is on a rise as
many Nigerians are turning to this option in order to relive their symptoms and improve their
overall holistic well-being.#The CAM (Complementary and Alternative Medicine) spectrum
comprises of wide and varied set of therapies and treatments that are not usually part of
conventional medicine but are rather influenced by cultural traditions, beliefs and personal
experiences.#These may size herbal medicine, pressure therapy, acupuncture and prothrombin,
spiritual healing and various mind-body rehabilitations among others.
The fact that the CAM make up is high among rural dwellers with musculoskeletal pain
demonstrates the significance of looking into the various ways and modes utilized during
treatments in these settings.#In addition, the traditional healing way of traditional Nigerian

Extending CAM applications beyond large cities to rural populations in Nigeria calls for
accessibility and affordability, including the CAM couplet.#The hospitals in suburban and rural
areas of the country often lack such vital services or may ask for way too much money, so the
only option the residents may have is to look for cheaper and more convenient alternatives.
CAM methodologies including those based on local materials and ways of doing things are

community.
Effectiveness of CAM perceived is the other parameters that affect individual's decisions in the

2.0 Objectives.
Getting acquainted with the scope, modalities as well as the factors causing the use of
complementary and alternative medicine system (CAM) is a necessity for research aimed at
understanding the prevail of CAM amid brown farmers with musculoskeletal pain in rural south-
western Nigeria.#A blended measurement technique that will use both quantitative and

3.0 Methods.

adequate representation and generalizability of findings.#The participants were chosen randomly
using probability-based sampling methods to avoid bias and affirm the diverse flavor of people

Data Collection:

structured on the basis of the literature review that we had done and feedback from the experts in

Participant:

were able to provide rich insights into the phenomenon in their own experience.#Criteria for

Data Collection:
We interviewed the selected representative subjects using a semi-structured method to encourage

questionnaire that was developed which allowed participants to answer the open-ended questions

Data Analysis:
The qualitative data obtained via interviews were analyzed through a process called thematic

data and then proceeding to code transcripts, look for themes, and interpret the findings.#Data

Ethical Considerations:
Informed Consent:

Recall Bias:

and interviews during the research provided not only numbers on CAM usage but also their
underlying roots, which allowed researchers to get insights into characteristics of the CAM user
population and the cultural and social context in which the decision to use CAM are
made.#Ethical consideration was paid special attention as research continued to aid the
participant rights and well - being.#Despite of some limitations of research, the results of study
are significant for generalization of findings in the specific area and so can be used for setting up
healthcare policies and practices in similar environments.
Participants:
The subjects for research were from a community in the coastal zone of southwestern Nigeria,
the locals largely being subsistence farmers from the hinterland.#A total of 300 stooped peasant
farmers were picked through random sampling to form the study population for the survey
analysis.#Furthermore, one-third of the participants involved in the study (these are all those who
used the complementary and alternative medicine) were chosen for in-depth qualitative
interviews to shed light on the varied experiences people have when using complementary and
alternative medicine.
Sampling Method:
The sampling process were that it was to delineate the population between just diversity not
everyone.#Thus the non-probability sampling technique of purposeful random sampling was
employed to select the peasant farmers who reported experiencing musculoskeletal pain.#The use
of this technique enabled us to reduce subjectivity and enhance the extra-palatability of our
results to the farmers of the peasant class in the whole community in the rural areas.
During the qualitative phases, the intentionally initial selection of people was made so that they
would provide a broad data set for the intended study.#Criteria for selection were age, gender,
social status and CAM type. On this basis, people who use different methods of CAM were
selected.#This moved allowed scientists to collect various perceptions and stories of individuals,
which had been validated and were now more explored.
Inclusion Criteria:
Participants included in the study met the following inclusion criteria:
1. Vet within the agrarian community of southwest Nigeria that are basically people from a
community that supports a peasantry farming system.
2. Individuals, suffering either from headaches or joint and muscle pain, regardless of whether
the pain was acute or chronic.
3. Readiness for study participation and their ability to give an informed consent.
Exclusion Criteria:
Those who were deemed to be ineligible for the program and those who expressed severe
disinterest in participating were barred from the study.#The aim was to make sure that the
specified criteria is met by all the participants and they are duly informed of the research
objectives, procedures, and their right to decide without any coercion or consent.
Data Collection:
In support of the research, quantitative data was obtained with the aid of structured
questionnaires directed to the participants.#The questionnaires were constructed in such a way
that they would allow the applier to gain information on different aspects like the CAM
utilization among the peasant farmers with musculoskeletal pain.#Subjects were asked about their
individual demographics, musculoskeletal pain characteristics, CAM-use history (e.g., location,

Data Analysis:
Structured questionnaires were used to collect quantitative data for which descriptive statistics
were employed in a computational manner.#I did this by looking at all the research done in this
area and presenting them graphically to help me to see the level at which CAM is being used
among peasant farmers and also to determine the demographic factors that drive the use of CAM
in my community.#To attain an accurate and reasonable data results, the software of statistics
such as SPSS and R in data analysis can come in handy.
In a bid to actualize the qualitative data, the data obtained via the semi-structured interviews
were analyzed using thematic analysis (Sundaresan, Krishna & Krishna, 2018).#The methods
applied were delimited to selecting the relevant themes and patterns among the farmer’s CAM

and categorizing them to look for key understandings. Qualitative data management platforms

4.0 Results:
The study results highlighting the prevalence and routes of use of complementary as well as
alternative medicine (CAM) among peasant farmers having musculoskeletal pain in the home
and rural areas in the South-Western State of Nigeria.#By classical methods of mixed studies
which are based on questionnaires and interviews, the study would give us valuable view about
the CAM followed by this population.
4.1 Prevalence of CAM Use:
Providing consistent and measurable water quality standards is crucial in monitoring and
modifying current conditions of surface and groundwater systems.
The questionnaire part of the survey found an inexplicably high use of alternative medicine
among the farmer group dealing with the problem of arthritis.#A total number of 300 individuals
in the survey reported closely applying one or more integrative approaches for the purpose of
treating their condition(s).#Such data shows CAM as a leader in healthcare delivery among the
community involved in the study, which may also be of great importance for those who are
trying to relieve their muscle and skeleton pain.
In this regard, the aging farmers recognize the effectiveness of these alternative healing
approaches by the extensively-reported cases and clinical researches.#Although traditional
healthcare is accessible to the participants, the significant number of participants still consider
CAM modalities to be their desire, thus indicating the strong cultural fixation of traditional cure.
4.2 Modes of CAM Utilization:
This means that only those with higher incomes, typically those with high-paying jobs or who
benefit from unequal inheritances, will be able to efficiently access healthcare and improve their
quality of life.
The in-depth interviews supplied the knowledge regarding the other roles CAM modes and
therapies played in the treatment of musculoskeletal pain by peasant farmers.#Herbal medicine
was the most utilized CAM modality by the majority of participants, due to the fact that it was
the inherited culture of using herbs in medicines that was firmly embedded in Nigerian
culture.#Peasant farmer families, which were big, used native plants and herbs which are local for
their remedy and actually took the pain away. So they added them to their diets and used them to
enhance their health.
The most popular of the CAM modalities involved massage therapy that participants
employed.#They were intrigued by the fact that the exercise that helped to relax tight muscles,

question.#Different from the common perception, which dictates that hunger and pain are the
inevitable truth of the lives of the peasant farmers, the peasant farmers attempt to cope with
sickness by blending aid therapies and health seeking behaviors. Through this strategy they are
successfully able to address the multifaceted nature of musculoskeletal issues and optimize their
well-being.
Implications and Interpretations:
The fact that the peasant farmers who complained about musculoskeletal pain preferred the
CAM done by the healers indicates how vital it is to get health systems to consider and
incorporate traditional healing methods.#Natural medicines, rub down techniques, and traditional
bone setting were basic modalities that were used in the quest for CAM (Complementary and
Alternative Medicine), reflecting deeply the cultural norms, behavior, and medicine seeking

Limitations and Future Directions:

4.3 Factors Influencing CAM Use:
The application of additional and alternatives medicines inside of peasant farmers with
musculoskeletal pain in the rural South-Western Nigeria can be determined by a wide range of

problem in Nigeria.#The peasant farmers may have to travel for a long, face access transport and
finances challenges which in turn impede their visit to central public healthcare facilities.#In
comparison, Complementary Alternative Medicine modalities usually are found more easily and
reached out in the local district.#Herbalist therapists are located close to the peasants, offering a

be attracted even without their knowledge of the particular treatments.
The elements of the interest of CAM among peasant farmers of Musculoskeletal pain in India of
the rural South-Western are multifaceted and are deeply threaded with cultural, socio-economic,
and healthcare considerations.#Apart from various other factors, the beliefs and value, ease of
access, affordability, the effectiveness, and the dissatisfaction towards the regular health services
are some of the reasons behind the pronounced use of complementary and alternative medicine
amongst the study community in our area.
The background knowledge, the understanding of the culture, and economic socio-demographic
dimensions that affect CAM use by peasant farmers was provided by the qualitative
data.#Musculoskeletal pain treatment practices originated in traditional medicine, effectiveness
approached subjectively, and a mind, body and spirit viewpoint on wellness came out as the
main themes identified in the study as CAM became an integral part of addressing the healthcare
needs of the target population.
