Client Background
Narrative description of client’s background and school experience (general education or special
education, any other information known):
Adan is a 16-year-old student who is in the 11 grade at Robbinsville High School. Adan is in general
th
education, but does have an IEP(Individual Education Plan) with a primary disability of OH(Other
Health Impairment) for Ehlers-Danlos Syndrome. This causes hypermobility and pain in his joints.
Adan also has a classification of “medically fragile” because of this syndrome. Adan’s area in needs as
stated in his IEP is Reading. Adan is a very bright student who is kind and respectful to his teachers
and peers. He tends to sit alone in class and does not engage with others very much. When reading or
working on assignments, he tends to work alone unless he can work with one of his friends. He also
has a strength in math and enjoys taking the Automotive CTE pathway at RHS. Adan scored an overall
reading score of 602(Grade 7) on his latest Iready assessment. Adan scored in each domain as follows:
Vocabulary – Early 11(662), Comprehension Overall – Grade 5(564), Literature – Grade 5(576),
Informational Text – Grade 4(556). Adan’s Lexile Reading Measure was 1075L which is in the 975L-
1125L Lexile Range.

Narrative description of test behavior and explanation of test results (250-300 words):
I gave Adan all the grades word list from Form A. I began Adan with List A Kb. Km, and Ke he got
all correct and very fast. Next, he completed List A 7141 and 8224 and got all words correct for
20/20 o n both. The next was 3183 and he got all right quickly. The next list was 5414 and Adan
got 19/20 and was noticeable upset for missing one. The next three list were 8595, 6867, and
3717 which he got 20/20 and was proud of getting them all correct. He completed list 8183 and
got 19/20. He started to get a little frustrated with the list and was trying to rush thru them so he
would be done. On list 4959, he got 19/20 with only missing one word. On list 1047 he got 14
correct with 2 analysis which got him a total score of 16. On list 1187 he got 16 correct and 3
analyses for a total score of 19. And on list 1296 he got 15 correct with 4 analyses for a total of
19. He was frustrated with his scores, but I encouraged him and we began the graded passages.
The first passage he read was A 6867 titled Volunteer. He got all questions correct with only 4
total miscues and was able to retell me the story accurately. The next was 3717 titled Blackout.
