Scriptural Foundation
The Great Commission (Matthew 28:19-20) – “Therefore go and make disciples of all
nations, baptizing them in the name of the Father and of the Son and of the Holy Spirit,
and teaching them to obey everything I have commanded you. And surely I am with you
always, to the very end of the age.”
The New Commandment (John 13:34-35) – “A new command I give you: Love one
another. As I have loved you, so you must love one another. By this everyone will know
that you are my disciples, if you love one another.”
God’s Call To Disciples (Mark 8:34-35) – “Then he called the crowd to him along with
his disciples and said: “Whoever wants to be my disciple must deny themselves and take
up their cross and follow me. For whoever wants to save their life will lose it, but
whoever loses their life for me and for the gospel will save it.”
The Parable of the Lost Sheep (Luke 15:1-7) – “Then Jesus told them this parable:

The Fellowship of the Believers (Acts 2:42-47) – “They devoted themselves to the
apostles’ teaching and to fellowship, to the breaking of bread and to prayer. Everyone was
filled with awe at the many wonders and signs performed by the apostles. All the
believers were together and had everything in common. They sold property and
possessions to give to anyone who had need. Every day they continued to meet together

Use Your Spiritual Gifts (1 Timothy 4:14-16) – “Do not neglect your gift, which was
given you through prophecy when the body of elders laid their hands on you.”
Humble Service in the Body of Christ (Romans 12:3-8) – “For by the grace given me I
say to every one of you: Do not think of yourself more highly than you ought, but rather

The Importance of Love (1 Corinthians 13:13) – “And now these three remain: faith,

Call to Teach Renewed (2 Timothy 2:1-2) – “You then, my son, be strong in the grace that
is in Christ Jesus. And the things you have heard me say in the presence of many
witnesses entrust to reliable people who will also be qualified to teach others.”
Call to Maturity (Colossians 1:28-29) – “He is the one we proclaim, admonishing and
teaching everyone with all wisdom, so that we may present everyone fully mature in
Christ. To this end I strenuously contend with all the energy Christ so powerfully works

The Importance of Scripture (2 Timothy 3:16-17) – “All Scripture is God-breathed and is
useful for teaching, rebuking, correcting and training in righteousness, so that the servant
of God may be thoroughly equipped for every good work.”
The Importance of Faith (Matthew 17:20) – “He replied, “Because you have so little
faith. Truly I tell you, if you have faith as small as a mustard seed, you can say to this
mountain, ‘Move from here to there,’ and it will move. Nothing will be impossible for
you.”
Be a Living Sacrifice (Romans 12:1-2) – “Therefore, I urge you, brothers and sisters, in
view of God’s mercy, to offer your bodies as a living sacrifice, holy and pleasing to God
—this is your true and proper worship. Do not conform to the pattern of this world, but
be transformed by the renewing of your mind. Then you will be able to test and approve
what God’s will is—his good, pleasing and perfect will.
The Amazing Power of Worship (John 4:23) – “Yet a time is coming and has now come
when the true worshipers will worship the Father in the Spirit and in truth, for they are
the kind of worshipers the Father seeks.”

Core Ministry Values
The Great Commission (Matthew 28:19-20)
oWe must go out to where they are and share the great news that is Jesus Christ!
We must do this with Christ central, with prayer, and with love
Christ came to where we were and fought for us and we must share that
great news!
The New Commandment (John 13:34-35)
oIn everything we do we must remember to love, this is important and central to
living out God’s commands and being a disciple
God’s Call To Disciples (Mark 8:34-35)
oGod’s call to be a disciple of him is three-fold
Deny yourself – We must strive daily to deny ourselves!
We do this by putting God’s commands and desires before my own
Take up my cross – We must follow after him, no matter the cost!
He may call us to step outside of a comfort zone or even the
country, but whatever the call and whatever the danger we must
answer and follow
Follow after me – We must continue to follow after Him and become
disciple-makers ourselves
Following after Him is a continual call; it is not a short-term thing.
It requires us to give Him everything including our lives

oGod cares for the lost and will do whatever it takes to bring them to Him.
In the same manner we must love those that are lost and live a life that
emulates Christ to them
We must rejoice when they return to our churches and not judge, our doors
must be open to all

oWe must, as a church, have one common purpose and that is to be the bride of
Christ and to practice Koinonia (Community)

oGod has gifted each of us with a special and unique gift and we must use that gift

GROW IN OUR KNOWLEDGE OF GOD AND GOD’S WORD…
oThis is an important part of building up our relationship with God.
By this I don’t mean just memorizing scripture or for that matter the entire
bible.
Memorizing God’s Word is important to our everyday walk as
believer but it does us no good if we don’t apply it!
PERSONALIZED THIS KNOWLEDGE BY LIVING IT OUT…
oSo learn all you can about how
The shepherd leaves 99 sheep behind in order to find that one lost sheep...
and realize, you are supposed to go bring that ―lost sheep back into the ‖
flock.
Love, how love is patient... and realize, you need to be more patient with
your family members, friends, even pastor!
You cannot serve two masters, either it is God or it is Money (not both)...
and realizes, you are supposed to choose God above all else.
Good behaviors, bad behaviors. It’s a choice you have to make as
you grow up.
AN INCREASING INTENTIONALITY OF TRUSTING IN AND OBEYING CHRIST
IN ALL AREAS OF LIFE…

The overall behavioral goal is to be Christ-like disciples that seek His face fervently in:

Vision Statement
Our vision with our students is to change our world by developing Christ followers who love
God and love people. To foster students to a life of discipleship which answers Christ’s three fold
call given to us in Mark 8:34-35. We hope to see a resurgence of students sold out to Christ and
willing to give it all up and follow after Him no matter the cost! We are committed to building a
gathering of high school students sold out for serving Christ and reaching their generation with
the love and message of Jesus Christ by holding to the following values:

The group that I am writing this discipleship plans for his the Harmony Baptist Church
high school ministry, Life Line Student Ministries. I grew up in this ministry and it has meant a
lot to me throughout the years. The one thing this ministry has been missing is a curriculum in
place for discipleship as there have always been a core group of students that have desired more
out of their walk with Christ. Harmony Baptist Church is located in Avon, Indiana, which is ten
minutes outside of Indianapolis. The youth group is made up of students from many different
high schools but there are four that make up the majority: Avon High School, Brownsburg High
School, Plainfield High School, and Danville High School make up this youth group. This group
is made up of a lot of different social and economic backgrounds. A majority of the students are
mainly freshman and seniors, which makes a very unique group. This group participates in a vast

number of ministry opportunities within the community and practices in many of the essential
aspects of ministry. A discipleship ministry will greatly benefit this group of students, this
church, and the community as a whole.
Action Plan
We will get from point A to point B by using a small group method with a 5/1 ration of
students to teachers. We will meet weekly at leader’s homes and discuss different aspects of what
it means to be a disciple. The areas we will go over will be: Abide in Christ, Live in the Word,
Pray in Faith, Fellowship with Believers, Witness to the World, and Ministry to Others. I will
also be doing a follow up with the different leaders each week do answer there questions and go
over any question or areas of importance for there individual group.

oWhat is God’s call for me?

oEach module will be two months long
oThe program as a whole will take a year to complete
oThe program will start after a church wide series on discipleship
The youth group will focus on discipleship
oEach month I will have a sit down meeting will all the leaders to get a progress
report on there groups and answer any questions
oAfter the program as finished (1 year) we will end on a mission trip
After which we will repeat the process with new students that desire to
learn what it means to be a disciple
Personnel
oThe personnel will be made up of adults that either already volunteer with the teen
group or those that desire to teach the students what it means to be a true disciple
oThere will need to be one leader for ever five to six students depending on amount
of students that sign up
The ideal group will be 5/1 student to teacher so that it is intimate enough
for true growth to take place but also relationships to take place
oI will walk the leaders through the next months curriculum at each monthly
meeting
oThe leaders and I will have a weekend retreat to prep air for this discipleship
program

Self-Assessment
The five essential activities are; spiritual formation, education, discipleship, focus of
ministry, and worship. I believe that I incorporated each activity into this discipleship program.
When it comes too spiritual formation I made sure that the student was taken from step a to step
b with care and compassion as they learn what it means to be a disciple. The next step, education,
was included in great detail when it came to the curriculum itself. The third active, discipleship

last two activities focus on ministry and worship was heavily included in the curriculum. I think
that I did a good job on including the essential activities and if I had to grade myself on a scale
from one to five, I would give myself a four.
