Journal Question:

As addressed in this week’s readings, population health rest on four pillars which include care management, quality and safety, public health, and health policy. Please provide a cogent discussion on why a population health approach is needed to promote health and wellness and prevent disease. In addition, within the construct of the social determinants of health, how do the four pillars of population health work together to improve population health? Lastly, please provide an example from your current or previous practice and experience based on the four pillars of population health.
