1. Human services are delivered to populations in a variety of settings, including rural areas, industry, the military, schools, and other community-based settings, all of which increase the number of clients who can be reached.

2. Schools have become a site for service delivery, as teachers and human service professionals share the same clients. The trend toward providing more services in community-based settings and less in institutional ones is helping to serve clients in the least restrictive environment.

3. Community-based services were first used with clients with mental illness who were deinstitutionalized; now, populations receiving community-based services include those in the criminal justice system, the developmentally disabled, and the elderly.

The barriers to service delivery in rural areas include shortages of workers, the large geographic

distances between clients and providers, the cost of services, confidentiality issues, and

coordination of care.

4. Technology has begun to change the way human service professionals meet their responsibilities, particularly in the areas of communication, information management, service provision, and professional development. Communication avenues used by human service professionals include e-mail, Listservs, the World Wide Web, cellular phones, and teleconferencing. Computer technology is available to help human service professionals manage information and use data to make decisions about planning programs, establishing costs, and reporting.

Technology is being effectively used to provide services to clients, including diagnosing and assessing, teaching new skills, and assisting those with disabilities or impairments.

5. The increased use of managed care organizations is dramatically changing the delivery of human services through the use of external reviews, the demand to develop standards of best practice, the need for better information management, and an emphasis on a continuum of care.

Managed care standards, developed for accountability in health and human services, provide

needed services efficiently and effectively.

6. Among the trends in the United States that will influence future human service delivery are the growth and change of its elderly population, the increasing diversity of clients and professionals, the increasingly active participation of clients, and the development of new and, in some cases, nontraditional skills.

Advocacy- speaking out on behalf of clients or those who cannot speak for themselves. Support

for or recommendation of a particular cause or policy.

Advocacy seeks to ensure that all people in society are able to: Have their voice heard on issues that are important to them. Protect and promote their rights. Have their views and wishes genuinely considered when decisions are being made about their lives.

Americans with Disabilities Act- The Americans with Disabilities Act (ADA) prohibits discrimination

against people with disabilities in several areas, including employment, transportation, public accommodations, communications and access to state and local government' programs and services.

Ethnicity- the fact or state of belonging to a social group that has a common national or cultural tradition. Diversity- the practice or quality of including or involving people from a range of different social and ethnic backgrounds and of different genders, sexual orientations, etc.

Diversity is the range of human differences, including but not limited to race, ethnicity, gender, gender identity, sexual orientation, age, social class, physical ability or attributes, religious or ethical values system, national origin, and political beliefs. Racial diversity conveys the idea that a relationship exists between race and social experiences, knowledge and practices.

Continuum of Care- is a concept involving an integrated system of care that guides and tracks patient over time through a comprehensive array of health services spanning all levels of intensity of care. Describes an integrated system of settings, services, professionals, and levels of care and services. The level of treatment is appropriate when needing constant supervision, medical intervention and therapy.

Mental Health Patient Bill of Rights- A patient's bill of rights is a list of guarantees for those receiving medical care. It may take the form of a law or a non-binding declaration. Typically, a patient's bill of rights guarantees patients information, fair treatment, and autonomy over medical decisions, among other rights.

Models of Service Delivery HUSR 108S- Introduction to Human Services

Three different models represent orientations in service delivery: the medical model, the public health model, and the human service model. Underlying each of the three models is a set of philosophical assumptions that guide the delivery of services and shape beliefs about the causes of problems, their treatment, and the role of the professional in-service delivery.

The medical model views the person coming for help as a patient whose problem is

diagnosed as a disease or sickness and treated by a physician or service provider who prescribes a treatment or cure for the ‘‘disease.’’ At the turn of the 20th century, Mary Richmond, author of Social Diagnosis, used the medical model to describe social casework.

Until the end of the 19th century, insanity was socially defined, but when psychiatry emerged as a medical specialty, insanity became a medical label. Psychotropic drugs are now among the most widely used treatments for mental disorders. Psychopharmacology, the study of the effects of drugs on mental health, is an area of interest to human service professionals.

The public health model, like the medical model, focuses on individuals and larger populations; it attempts to solve many of society’s social problems. Communicable diseases, poor sanitation, and lack of medical knowledge have been primary community health problems since ancient times. Laws such as the Massachusetts Poor Law of 1692 in North America and the Public Health Act of 1848 in Britain initiated efforts to improve public health conditions.

The U.S. Sanitary Commission was the first major public health group in the United States. Its efforts were primarily preventive, alerting the public to the benefits of preventive sanitary measures.

A primary focus of the human service model is to provide services that help individuals solve their problems, including a consideration of client problems within the context of their environments.

The primary method of treatment is problem solving, a process focused on the here and now that encompasses three phases: problem identification, decision making, and problem resolution.

The human service philosophy emphasizes both the responsibilities of society and the importance of working with the whole person. Themes characteristic of human service delivery are its generic focus; accessible, integrated services; problem-solving approach; treatment of the whole person; and accountability to the consumer. Using models is a helpful way to identify the different approaches to clients’ problems.

Summary of Terms:

Client –someone in need of help because they lack either the necessary abilities or the capacity to

help themselves and thus need the specialist knowledge and skills of the social worker. Clients are

active participants in the human service model, making decisions, taking action, and accepting

responsibility for themselves

Electroshock therapy - This treatment involved administering an electric shock of 70 to 130 volts to

the brain, leaving the patient unconsciousness and/or in convulsions.

Human service model – treatment of client by working with both the individual and the environment

that is around him.

Medical model- a model of health which suggests that disease is detected and identified through a

systematic process of observation, description, and differentiation, in accordance with standard

accepted procedures, such as medical examinations, tests, or a set of symptom descriptions.

Neurosis - a relatively mild mental illness that is not caused by organic disease, involving symptoms

of stress (depression, anxiety, obsessive behavior, hypochondria) but not a radical loss of touch with

reality.

Problem solving - a process focused on the here and now that maximizes the identification and use

of client strengths.

Psychiatry - an area of medicine involving the study, diagnosis and treatment of mental health

disorders. A person's mental health is influenced by a combination of factors that are both specific to

an individual as well as related to interactions with society, community, and family.

Psychoanalytic method - in this method, the clients/patients share all thoughts with the therapist. The

therapist interprets this material to explain to patients the nature of their repressions and the influence

of these repressions on their present problems.

Psychopharmacology - which focuses on ‘‘the psychological effects of drugs and the use of drugs to

treat symptoms of mental and emotional disorders’’

Psychotropic drugs- psychotropic describes any drug that affects behavior, mood, thoughts, or

perception.

Public health model- a concept with currency in many disciplines, including health, education and

welfare. It is an epidemiological model that attempts to prevent or reduce a particular illness or social

problem in a population by identifying risk indicators.
