This assignment has 3 steps.

· Find your states IEP form and the sections you need to complete.

For this assignment you will want to find the IEP sample form for your state. (If you have trouble finding your form please reach out to me.)

Here are two forms that you might use if you cannot find your form:

National Association of Special Education Teacher (collection of IEP templates from states)

You are going to complete these sections of the IEP form based on your students case study.

Include the following on your student's IEP:

· Student information

· Student performance and prior learning experience

· Special instructional considerations

· Annual goals and benchmarks

· Supplementary aids, such as assistive technology and related services to support varied student learning needs

· Communication and language demands

· Special education placement

· Special services

· Transition services

· Modifications and accommodations for the learner

· Transferral of rights

· Recommended instructional and/or behavioral interventions

· IEP development team

2.Read the Report so that you can complete the IEP form.

To complete the IEP form you are going to need to read the students report.

I recommend you:

☐ Read through the Student Report carefully.

☐ Use the WISC-IV Explanatory Chart and the Woodcock-Johnson Explanatory Chart to help interpret the scores.

☐ Look at Chapter 2 in readings for this week.

☐ Compare your student’s scores to the Average Standard Score (note how far above or below the student falls).

☐ Watch the video walkthrough of Derek’s FIE reports. * This video shows some resources from another course but process will be similar for this assignment in this course.

3.Complete the required sections of the IEP form.
