Week 2: Content Application Assessment

Criteria A=93% to 100% Distinguished B=84% to 92% Proficient C = 75% to 83% Basic D/F= 74% or below Non- Performance

Criterion 1: (55 Points/55%) Uses the Community Assessment Guide to collect community/neighborhood level data for the assigned categories; Community People (Demographics and Health), Physical Environment, Health, and Human Services. Uses comparative data from county and state where indicated. Data is accurately represented and complete. Assessment includes an introduction and a conclusion.

51 to 55 points

Uses the Community Assessment Guide to collect data for the 3 assigned categories; Community People, Physical Environment, Health, and Human Services. Uses comparative data from county and state where indicated. Data is accurately represented. Assessment includes an introduction and a conclusion.

46 to 50 points

Community assessment guide includes all required data and comparative data but may need more detail/explanation. Assessment includes an introduction and a conclusion.

41 to 45 points

Community assessment guide is utilized but some data and comparative data are incomplete or county, state and federal data are used too frequently for primary data. May be missing an introduction or a conclusion.

0 to 40 points

Uses the CAG to display data but a significant amount of data is in the wrong category or too broad, OR CAG is not used and data is not accurate, relevant /connected to the community. May be missing both an introduction and conclusion.

Criterion 2: (25 Points/25%) Displays all data required in the Community Assessment Guide (CAG) in a clear, concise, and organized manner. Integrates and applies information/ uses skills gained from assigned readings, course content, and discussions. Uses the comment section in the CAG to provide context or to make clarifications where needed.

Data is mostly disorganized, or unformatted, lacks clarity and/or multiple categories are incomplete throughout. Majority of categories are incomplete and explanations are not utilized when needed.

Criterion 3: (10 Points/13%) Includes at least one reference from this week’s learning and one new, current (within 3 years), peer-reviewed scholarly source from the UIC library that is not listed in the course material. (This is the minimum; other references can be utilized to provide more support.) References are related to content.

Content included is mostly synthesized from the resources but some content lacks connection and/or some statements in the paper do not match references.

0 to 7.4 points

Includes only 1 required resource and/or some statements or data in the paper do not match references. Content is not synthesized and/or is simply copied and pasted with minimal evidence of connection to resource or assigned resources are not utilized as required or references are not included.

Criterion 4: (10 Points/13 %) Writing is clear, with appropriate spelling, sentence structure and punctuation. Data is cited. In-text citations have a corresponding reference listing. Complete and error-free APA formatting is utilized for both reference (s) and in-text citations. Direct quotations are not used.

4-5 formatting, writing, grammar, or APA errors and/or in-text citations may not be present or match reference (s). Some data points not cited. Direct Quotations are not used

0 to 7.4 points

More than 5 grammar, spelling, writing, or APA errors and paper is very difficult to follow due to structure/formatting issues and/or direct quotations are used frequently. In text citations are used inappropriately, infrequently or completely omitted where required. Data is rarely cited or not cited at all.
