To address health issues that affect a whole population or groups within a population,
community health nurses must apply the steps of the nursing process (assess, diagnose, plan,
implement, and evaluate) to an entire community. The community health assessment model
provides a framework for this approach to health promotion (NURS 362, 2025).
Community Health Assessment
Community assessments are essential tools for community health nurses. The community
health assessment model uses a variety of resources and methods of data collection to identify
areas of need so community health nurses and organizations can plan appropriate health
interventions for higher risk or underserved populations within the community (NURS 362,
2025).
The community of Monmouth-Independence, Oregon is made up of two neighboring
small towns which share many resources. Both towns are a mix of rural and suburban areas.
Monmouth is home to Western Oregon University. Independence is bordered by Monmouth on
one side, and by the Willamette River on the other side. Both towns are otherwise surrounded by
farmland.
