Research health care providers (physician's office, clinic, or  hospital) in the Minnesota area to find one that publishes their quality-data for  the public to view (it can be online or in print). \

Write a 2 page paper in APA format that describes:

1. The quality data the organization/provider presents (such as mortality rates, readmissions).
2. The source of their data (e.g. survey tools, quality measures, or projects).
3. The time period of the report.
4. How the organization works to improve the quality of care.
5. If the provider received any special recognition or achievements based on their quality efforts. 
6. Why specific data were of particular interest to you. 
7. Other quality data that you would find beneficial to know about  this organization/provider and support why it would be useful to you as a  consumer of health car
