**Similar to the first two discussions, review a scenario of your choice. This will be added to your final action plan at the end of class.**
Listed below are topic(s) I would like for each of you to discuss throughout the week. Refer to the textbook, weekly materials, and/or the GMC library when researching the topics for this Discussion Forum. Put your writing into your own words, do not copy directly from the source. If you incorporate external resources in your posts be sure to cite them properly. Lastly, review how to paraphrase and quote resources before you begin posting.
· Please select one of the scenarios to answer. You should provide a well thought out response to how you would handle the situation. The answer to this scenario will be the basis for part of your final project. You will respond to two classmates who selected a different scenario than the one you picked. Once this forum is completed, you will be able to take your response from here and edit it to include any suggested changes or additions to your "action plan" to complete your final project.
· Scenario 1: You are a first responder in a Southern California community that has experienced a major mudslide. Many homes are covered with sediment or are damaged extensively. Volunteers are on scene to assist but time is running out. What search and rescue concerns do you have and how do you address them?
· Scenario 2: A bus carrying 40 high school students has overturned on a freeway in Oklahoma. Injuries range from minor to major and some students have died. How can triage be applied in this situation?
· Scenario 3: You are a member of a nonprofit organization in a community that has just experienced a major disaster. Hundreds of people have died and others have lost homes and possessions. Some victims are fighting with insurance companies to settle claims and others believe the federal government is not providing enough disaster assistance. What are your concerns about people in your area? What can you do to help them cope emotionally? Are there things you can do to prevent symptoms of PTSD?
· Scenario 4: While responding to a 3-alarm fire at an apartment complex, you become worried about the physical and emotional well-being of your fire department. What can you do to prevent critical incident stress? If some are affected, how do you help them afterward?

Biosecurity & Bioterrorism: Containing and Preventing Biological Threats
Chapter 7
Case Studies
Learning Objectives
Analyze biological events, natural, accidental, and intentional, with an eye toward understanding the challenges of detection and subsequent containment and mitigation efforts.
Discuss the significance of the six Case Studies presented in this chapter.
Identify opportunities and challenges in detecting an emerging disease.
Compare the differences in disease transmission from natural, accidental, and intentional biological events.
Compare the economic and societal impact from natural, accidental, and intentional biological events.
Key Terminology
Cluster
Cordon Sanitaire
Fomite
Case Studies
Anthrax; Sverdlovsk, USSR
Salmonellosis; Oregon, USA
Plague, Sarat, India
Amerithrax; USA
Ricin; USA
Ebola Virus Disease, West Africa 2014/15
Sverdlovsk, USSR
Occurred in 1979
Accidental release of Bacillus anthracis spores from a bioweapons production facility
Small amount released at night
66 deaths resulted
Soviet officials initially reported that the event was due to consumption of infected beef
Outbreak was not consistent with GI Anthrax
Yeltsin later admitted to the truth

Sverdlovsk- Discussion Questions
Could there be legitimate national security reasons for not disclosing the source of such an outbreak?
If there are reasons, what are the potential ramifications for recognition, containment and mitigation of the danger from the organism?
Salmonellosis; Oregon, USA
Occurred in 1984
Perpetrated by the Rajneeshee religious cult
Aimed at influencing local elections
Took nearly 2 years to determine the source of the outbreak
Department of Corrections
Foodborne - Discussion Question
The Rajneeshees incident occurred in 1984. However, had this event occurred after the 2001 anthrax attacks, do you think investigators would be so quick to discount an intentional attack? Many years have passed since the 2001 attacks; do you think our vigilance has a shelf life?
Plague; SUrat, India
Occurred in October, 1994
Natural outbreak of bubonic plague
Led to pneumonic plague cases
Government officials were slow to enact quarantine procedures
CDC was called in to “lend assistance”
Took many months to contain
Affected trade and tourism in India
Plague - Discussion Questions
What is cordon sanitaire and what does it have to do with this outbreak of plague?
Numerous bioterrorism exercises have focused on pneumonic plague – how prepared are we for widespread quarantine measures?
What obstacles would we encounter legally?
What challenges would we face logistically for a large outbreak of pneumonic plague?

Amerithrax, 2001
11
Amerithrax; USA
Occurred in fall of 2001 a few weeks after 9/11 al Qaeda attacks
Several letters entered the mail in Trenton, NJ – 22 cases, 5 fatal
Index case, Bob Stevens, was a photo journalist in Boca Raton, FL
Next two fatal cases were postal workers from Brentwood facility
The other 2 fatal cases - NYC and CT
12
Amerithrax – Discussion Questions
Based upon your knowledge of inhalation anthrax, how does the clinical presentation from the index case measure up with the incubation period and final outcome?
Why hasn’t it happened again?
In your opinion, was this an effective act of bioterrorism or the work of an amateur?
Ricin; USA
Fall of 2003 and February, 2004
Amateur bioterrorists with crude extracts of ricin sent letters to White House and Senator Frist’s office in the Senate Office Building
Another letter sent from the “Fallen Angel” recovered from a postal facility in Greenville, SC
Ricin – Discussion Questions
What is the risk to over reacting and carrying out containment activities every time a suspect item is discovered at government offices?
Why is ricin often used in acts of bioterrorism?
How easy would it be to do this?
How effective is this type of attack in creating casualties?
Ebola Virus Disease – West Africa 2014 – 2015/16
EVD Response
History of the Outbreak
Index case late December 2013 – small village in Guinea
Not recognized as EVD initially, spread to other villages and across national borders
WHO slow to respond
ETUs overwhelmed by summer 2014
Spread to US and Europe with limited spread
EVD - Discussion Questions
How can factors of politics and emotion be minimized during the investigation of an outbreak?
What have been the economic and social repercussions in West Africa due to this outbreak?
Is the developed world ready for VHF outbreaks? Why or why not?
With reference to the initial response, does it matter whether an outbreak is natural, accidental, or intentional? If yes, how does it matter and to whom? If not, why not?
What has the 2014/2015 EVD outbreak taught us about a few cases of VHF getting into the U.S. or any other developed country? Consider applying the criteria for Category A agents.

Chapter Summary
Case studies consistently point to a breakdown in systems
Recognition is often a problem
Post-event communications are difficult and often problematic
Containment measures sometimes warrant quarantine procedures that aren’t always feasible or practical
