I have to write an obituary

I need an word document obituary. you can just rewrite the one I have attached verbatim. Only adjustment should be the dates.  So the sunset dates (2 of them one on the front and one on the inside) should be December 12 2018. and the funeral date on the front needs to be Saturday December 22 2018. I have also attached a picture to use.
