Assignment Description

Step One: Visit the following website: https://www.msms.org/Portals/0/Documents/MSMS/Resources/For_Practices/PCMH/PCMH_Case_Studies.pdf

Select one of the 13 case studies provided to create a flow chart. Use a different case study then the one you used for W1A2 assignment.

Step Two: Using the information from the case study, create a process map using either a process diagram or a flow chart of the EHR implementation steps that were followed. Be sure to use appropriate chart symbols.

Step Three: Identify an area where improvement may have been made if the steps were in a different order. Create a new process map or flow chart using the new order.

Step Four: Develop a short summary (about 300 words) as to why you think this change would make a difference.

Step Five: Submit your two process maps or flow charts along with the summary for grading.
