Evidence based project

It is an Evidence based project. I have topic selected and PICOT question for the project. It is 8-10 page long project and requires primary source table for the references used. I have attached rubric and table. I also need plagiarism check. 

PICO(T) Question: In patients aged 65 years and older (P) admitted to rehabilitation following hip fracture repair, how does implementing an individualized fall prevention program incorporating hip precaution education, assistive device training, and environmental safety modifications (I), compared to standard fall precaution protocols — defined as routine measures such as bed/chair alarms, non-slip footwear, fall-risk identification bracelets, and hourly nursing rounding without individualized education or environmental modification (C) — affect the rate of falls and fall-related injuries (O), over a standard 15-day inpatient rehabilitation stay (T)?
