Centralized HIE models connect several participants like payers, providers, and public
health data sources by using a single common data pool. Its cost is highest than decentralized and
hybrid models due to diverse setup and maintenance needs. The decentralized or federated HIE
models encompass a collection of clinical data repositories that are situated remotely. The patient
data is not stored in a centralized and accessible location; instead, it is stored locally. Its
implementation time is lowest, and security is highest as compared to the centralized and hybrid
models. Hybrid HIE models combine the features of the centralized and decentralized models and
facilitate diverse functions like clinical analysis, medical research, and risk stratification. It is the
most efficient model as it combines the advantages of the other two HIE models (McGee & Ross,
2021). f
The directed HIE exchange is used by service providers to easily and securely send patient
information like patient referrals, laboratory orders, etc. The query-based HIE exchange is used by
providers for searching and exploring accessible clinical resources relating to a patient. The
consumer-mediated HIE exchange gives patients access to their health information, and thus they
are able to manage their healthcare virtually (What is Hie? HealthIT.gov, 2020).
HIE Model for Manchester Health Access Network (MHAN)
In the Memorial Hospital, the hybrid HIE Model would act as a suitable architectural model
as it would help to make the optimum use of data and information in the delicate health
environment. It would provide ample flexibility to achieve the desired interoperability while
staying within the budget. f The mode has been chosen for the MHAN network as it would facilitate
the maintenance of a centralized clinical data warehouse (CDW) that would be interfacing with
data from every local remote CDW. By using it, patient information could be easily accessible by
every member hospital member (McGee & Ross, 2021).

HIE Exchange for Manchester Health Access Network
An appropriate HIE Exchange for Manchester Health Access Network is the directed
exchange. At present, one of the main issues in the facility is the inability to share timely
information and coordinate the care of the patients. By integrating the directed exchange, the
service providers could directly send electronic care summaries like health problems, medications,
lab results, etc., to specialists while referring a patient. Thus, this exchange could enhance the
functional aspects of the hospital (McGee & Ross, 2021).
Recommended strategies for promoting interoperability programming standards
The proper promotion of interoperability programming standards is of paramount importance
while choosing the suitable HIE model and exchange. A number of recommendations have been
made that would ensure that the incorporated solutions are aligned with the relevant
interoperability programming standards:
• The hospital must modify its technical specifications relating to the new HIE system so that
the streamlined movement of electronic medical data can be facilitated (CMS proposes
modifications to the Promoting Interoperability Program. Healthcare IT News, 2021).
• Certified Health IT must be included who would play a proactive role to support top-quality
and high-value care and use of Health Information System (HIE).
• Memorial Hospital must establish the requirements relating to the electronic capture of
clinical data, including providing electronic copies of health information to patients.
