Chautauqua County (WNY) has a prevention agenda that is available
online to the general public. The agenda includes measured data that
shows how each of the four focus areas are performing. There is a
matrix available that lists the resources and the intervention strategy
for each of the four focus areas. The prevention plan and focus areas
are as follows:
* Access to quality health care - purpose is to increase health care
coverage across the county. The collaborating hospitals are Brooks,
Westfield, TLC and UPMC. The partners available to help with
providing resources include Chautauqua County health network,
local providers, Chautauqua County Dept. of Social Services and
Community Service Agencies. The intervention plan is for all
partnering organizations to refer uninsured or underinsured clients to
the NYS GetCovered Helpline. The helpline is designed to help the
general public get healthcare coverage through the state.
* Chronic Disease - purpose is chronic disease and diabetes
prevention and assistance. The collaborating hospitals are TLC,
Brooks, Westfield and UPMC. The partners available for help with
resources are Chautauqua County Health Network and P2
Collaborative of WNY. b The agenda for this prevention strategy is to
promote and assist consumer lifestyle changes by helping people
take control of their health care plan. b The Chautauqua County
Diabetes and Chronic Disease Prevention Task Force will assist in
increasing awareness of the diseases. The agenda is to increase
awareness to promote prevention and help with care needs
associated with diabetes and chronic disease.
* Infectious Disease - purpose is to focus primarily on influenza and
COVID-19. Collaborating hospitals include Brooks, Westfield, UPMC
and TLC health network. Resource partners are Chautauqua County
Health Network, Healthcare facilities, all partnering local schools and
community places for religious gatherings (local churches). This
prevention strategy focuses on awareness of COVID-19 and COVID-
19 prevention along with influenza prevention. Services offered
include free COVID-19 vaccinations and boosters and free testing
facilities. There are resources available to educate the general public
about the importance of vaccinations and provide personal hygiene
reminders that can help prevent disease. b All partners are available to
provide flyers and resources to the general public about infectious
disease prevention. The goal to make the public aware of how to
prevent infectious disease to stop the spread.
* Physical Activity and Nutrition - purpose is to educate the public
about programs that promote wellness through exercise and diet.
Collaborating hospitals include Brooks, Westfield, TLC health
network and UPMC. Resource partners include Chautauqua County
Heath Network, Community agencies and Chautauqua County
governmental agencies specializing in public health. The agenda is to
educate the general public on available wellness programs. These
programs promote aerobic exercise and good nutrition. There are
walking and bike path maps available for the county. The partnering
hospitals have nutritionists available to help people with a good diet
to fit their health needs.
As you can see our county has many resources available to help the
public get educated. Our county relies on local community efforts to
assist with educational tools. b Most of our local schools have an area
of the lobby dedicated to health education and disease prevention
(it's mainly pamphlets and educational newsletters). I think my
county does a good job of presenting the materials but I don't think
the general public takes advantage of the available resources.
