Interventions to Promote an Environment of Safety

Introduction
Mr. Joe Smith is a 75-year-old man recently suffered a left cerebrovascular accident. He
has right visual field deficits and residual weakness in this right arm and leg. He uses a walker
for mobility. He takes Metoprolol for his blood pressure and the physician started him on
Coumadin because he has recently been diagnosed with atrial fibrillation. Based on the case
study some actual and potential problems I have identified are high falling risk, bleeding due to
medications, and mobility impairment.
Nursing Interventions
One nursing intervention for Mr. Joe Smith would be to encourage him to have regular
eye exams. Annual Eye exams for the older adult including dilating your eyes are important
because the doctor can have a better look at the retina and optic nerve so they can identify any
potential problems and intervene early (McKinney, 2020). Second nursing intervention for Mr.
Joe Smith would be to urge him to remove all rugs from his home. Removing all rugs from his
home is important because it will lessen a potential risk for trips/falling in his home. For more
decreased falling in the home a third intervention would include proper lighting in the home so
Mr. Joe Smith can assess the area clearly in well-defined lighting. For the fourth intervention it
would include to encourage Mr. Joe Smith in having clean and clear floors. With clean and clear
floors, it would lessen the risk of him falling due to something on the floor or even for his walker
to get stuck on. The last three interventions are for reducing the risk of falling which is the
leading accidental death of older adults (Wilkinson, 2021, Ch. 24). Additional interventions for
Mr. Joe Smith would encourage him to soft pad corners of furniture, tables, counters, etc. Putting
soft pads on pointed or sharp edges would help prevent Mr. Joe Smith from bumping into them

and possibly causing bleeding since he is on blood thinners (Coumadin). With blood thinners
such as Coumadin the client is at risk for bleeding much longer since that medication is reducing
the development of blood clots (Wilkinson, 2021, Ch. 26). Although Mr. Joe Smith has mobility
impairments it is vital for him to move throughout the day such as small activities to promote
blood circulation and provide his body of movement. Which exercise has been shown to lower
the risk of hemorrhaging and boosting mental health (Shendre, Beasley, Brown, Hill, Arnett, &
Limdi. 2014). All interventions are important to help Mr. Joe Smith promote the safety of his
environment.
