A one page description of major practice experience activities completed to date is due with
each Practice Experience Log. The Practice Experience Log should be submitted as a separate
document. The purpose of this is to give your preceptor and faculty a synopsis of where you are
with identifying your problem at your site week 8 (supported in the research and professional
literature) and where you are with identifying the evidence base that will support your
intervention week 14. Each Project Status Update Assignment is worth 5 points.
Throughout the DNP program, please get in the habit of labeling the attached file with: “last
name.first initial. title of assignment.”
This makes it much easier for faculty to identify individual student’s work when they are
saving/grading them all on their computers.
Both logs must be signed and are graded pass/fail. Your preceptor should also mark Y/N as to
whether or not they want a teleconference with course faculty. Please follow the instructions
and guidelines for what should be included on the log and how to code those items as provided
with the practice experience content. Academic assignments do not count as practice
experience.
The one page Project Status Update must include the following:
Location of site / status of affiliation agreement when known
Name/credentials of preceptor when known
IRB selection (Chatham or outside institution) when known
Major activities to date regarding the problem identification (week 8) and evidence
identification supporting the intervention (week 14)
General ideas for a project design (week 14)
Any issues, concerns, or stumbling blocks experienced to date
This is an informal paper therefore no title page or references are needed.
