What is today’s goal?
Upon inspection, there are no signs of strabismus or nystagmus. Pupil size is
about a 4.
Eyelids are symmetrical w/ normal blinking patterns. Eyelashes are brown,
symmetrical, and evenly distributed, upper lashes curve out and up, lower
lashes curve out and down. Eyebrows are brown, symmetrical, and evenly
distributed. There are no lesions, edema, ptosis, tremors, or flakiness noted.
Lacrimal gland- palpate around eyes, no enlargement or dryness
present. Orbital area- no edema, sagging, drainage, lesions, or puffiness
present. Conjunctivae- clear and inapparent
Sclera- (pull eye lids down) white, no exudate or foreign bodies present.
Also, no pterygium, corneal arcus, or hemorrhaging present.
Corneal clarity- (shine light from sides of eyes) no scars, abrasions, ulcers, or
cloudiness.
Iris- clearly visible, brown bilaterally, and no

1. Snellen chart: for distance, I would have you read the lowest line you
can see. For example, if the top line is the lowest you could see then what
you can see at 20ft is what someone with 20/20 vision could see at 200ft.
2. E-chart & animal/abject test: for kids
3. Rosenbaum chart: for close up, 14 inches
4. 6 visual fields- pt. covers one eye, you cover opposite eye, use
bunnies, verbalize for other eye, saves time.
Cranial nerve # 3 (Oculomotor), # 4 (trochlear), & # 6 (abducens):
1. corneal light reflex: light on bridge of nose, look for even reflection.
2. Cover/uncover: cover eye, focus on distant object, then close object,
uncover eye. Eyes fixate and accommodate , not doing other eye to
save time.
3. 6 cardinal fields of gaze- pt. follows pen light, not doing other eye to
save time.
-Shine light directly in pt. eyes, do this twice. Eyes are consensual, Eyes
are PERRLA.
Ophthalmoscope
1. Lens clarity: clear
2. Red reflex: color around iris, present
3. Retinal color: yellow or pink background, no other pigmentation or
any lesions noted.

4. BV characteristics: Artery/vein ratio is 2:3, no nicking or
tortuosity present.
5. Disk: yellow to creamy pink color, 1.5mm in diameter.
6. Physiological cup: symmetrical with distinct margins.
7. Macula: yellow dot surrounded by a deep pink color; no
cloudiness noted.
