Alternate Assignment – Family Health History
Answer the following questions about yourself. You may answer in sentence form following
each question and submit the assignment as a WORD document.
1. When was the last time you and/or your parents (if applicable) had the following cancer
screens? I have not checked whether I have cancer or not and as far as I know, my parents
have done their screening as they are entering their mid fifty and it came out good.
Females: a. Pap smear (age 21 or older)
b. Self-Breast Exam
c. Mammogram (age 40 or older)
Male: d.Testicular Exam
e. Prostate Exam (age 40 or older)
Both: f. Colonoscopy (age 50 or older)
g. What is the purpose of these screenings and why are screenings important?
2. a. How often do you use sunscreen when in the sun? I am the type of person who would
apply is 24/7 whether it’s sunny or cloudy. I try to put sunscreen on every day.
b. When was the last time that you saw a dermatologist for a skin checkup? I never have
before.
c. Why are these issues important? Because it could cause skin cancer unconsciously.
d. Based on your sun history and your online search, are you at risk for developing skin
cancer? Not really, I am not at the risk of developing skin cancer.
2. a. Do people with whom you live smoke, vape or juul inside the home? No, not at all.
b. Are you around people who smoke, vape or juul? No, I am not around people who
smoke.
c. If yes, what increased health risks do you have because of this exposure?
3. a. When was the last time you had a yearly physical with blood work? This summer.
b. What things do you normally check when having blood work done? Whether I have
problems with my liver, kidney and have diabetes, HIV, anemia.
c. Why is it important to have blood work done? It is important to prevent possible
diseases.
d. What chronic diseases are identified commonly through having blood work done?
diabetes, HIV, anemia, cancer, coronary heart disease.
4. a. How many times per week do you participate in any activity that raises your heart rate
for at least 20-30 minutes? Once or twice, I like running. When I go out, I run at
least 30 minutes.
b. How many times per week do you participate in any activity that raises your heart rate
for at least 60 minutes? On average, I work out once, I try to do 3 times a week but
still managing myself doing once a week at least.
c. What impact does this have on your overall health? I can feel that I have more
energy than when I do not work out and it seems like I have to do exercise not only
to prevent diseases and to have health life but also to have actively energetic life.
5. What were you taught about health and a healthy lifestyle growing up? I taught I was fine; I
could eat anything because I was young but had a sense that managing what I eat and
exercise gives me some kind of benefits.
6. a. If taking any medications or herbal supplements routinely, do you keep a list with the
name, dosage and purpose with you, in case of an emergency in which you were
unable to communicate? No, I never thought those kinds of situations would
happen.
b. Where is this list kept? Probably good idea to keep it nearby product.
c. Who knows where this list is? Nobody, but if I had to assign, I would say my sister
and one of my closest friends.
d. Why is this important? In case of I need for emergency situations.
7. a. Have you taken and passed a first aid or CPR course in the last 5 years? No, I have
not taken.
b. If you have not, why haven’t you? I thought I do not need because it is really rare
to see where those types of situations would happen in my real life.
8. a. When you are at home, how many nights each week, on average, do your family
members eat the evening meal together? Since me and my sister study in the States
and my parents are in Korea, we don’t really have a chance to have dinner together.
But it happens when we go back during summer.
b. Do you sit at a table when you eat together. Yes, we do have our meals together at
the table when we eat.
c. What impact does this have on your overall health? I would learn table manners
from my parents and learn about the concept of healthy living style, learn correct way to
consume the food.
d. What do you plan to do with your family dinner practices? I try to have
conversations, with my parents and tries to draw positive reactions about it.
9. a. Would you say that God is a priority for you? Yes, He is my priority.
b. What evidence of this is there in your life? I try to take an action that pleases God, I
memorize bible verses to keep in His way, pray every day in the morning, and try not to
give up even though there are times when I fail.
c. Was this modeled in your home or is this something that you have had to develop on
your own? I would say some from my parents because I did not like that my
parents spending their time at the church not spending enough time with me when I was
young. But I know that because people told me they have prayed for me a lot. My devotion
habits grew by many people and friends from the church.
10. In relation to your present health behaviors, what conclusion could you draw about disease
prevention for you and decisions that you should be making?
Begin consuming multi-vitamins, exercise regularly at least three times per week,
apply sunscreen every single day and cleanse well after sunset, spend time with God
continuously.
