QEP DPRP: DIAGNOSTIC SUMMARY REPORT (DSR) TEMPLATE
Tutor (You): Gracie Frame
Client Name, Age/Grade: Hunter Kellogg, Kindergartener
Subject of Focus: Letter and Sounds
Assessment Administered DIBELS 8th edition
Summary of Scores
(Be specific)
When measuring the Letter Name Fluency, I used a marker to track the letters to help him focus.
Hunter tried to say the name of the each of the letters but was struggling and becoming frustrated
with himself and getting distracted. In the sixty second timeframe, Hunter was able to tell me 2
of the 10 letters. The first letter he recognized was C and he recognized it pretty quickly. When
he got to the letter D, he missed it. After that, he answered the rest of the letters as B and soon
became easily distracted and unmotivated to try. Hunter is in the red range.
Reading or Writing Level:
(Based on results of pre-
assessment. Note whether this is
above, below, or at grade level)
Hunter is below grade level.
Identified/observed strengths in
subject area of tutoring:
(List at least 2 with explanation)
1. Hunter admitted that he had no confidence in his answers because he simply did not know. I
told him I appreciated his honesty, and because he told me how he was struggling, I could find
ways to help him.
2. Activities and items that Hunter enjoys are used in the tutoring to improve his self-confidence
and skills.
Identified/observed weaknesses 1. Hunter hates missing the special times such as art and music.

in subject area of tutoring:
(List at least 3 with explanation)
These are the areas that were
“diagnosed” during pre-testing.
2. Because Hunter is doing his work in the hallway, it makes it hard for him to concentrate
because of the traffic and activities happening in the hallways.
3. Hunter needs help more than once a week. Only getting one hour of help a week is not
sufficient for the help Hunter needs.
Input that was received from the
client, parent, or teacher in
regards to the needs of the client:
(For example, are there specific
concepts they have asked you to
focus on?)
Teacher reported that Hunter is very hyper and has a hard time staying focused.
Based on the client’s personality
and interests, the following
activities/techniques will be
incorporated in lesson planning:
*Note activity ideas, technology
sources, etc.
1. Hunter loves Dr. Suess, so we will be using his books to practice and learn.
2. Hunter loves to play with play dough. We will use play dough to learn our letters and practice
writing letters of the alphabet and he will use the play dough to make the letters.
3. Since Hunter loves the High Ho the Cherry Oh board game, I will incorporate a similar
digital game that Hunter can play on his school ipad that helps him with his letter recognition
skills.
4. I will put together a beginning sound match game. There will be a mat with images from Dr.
Suess books. Hunter will have to use plastic alphabet letters to match the pictures with the
correct alphabet letter that matches the beginning sound.
5. Since Hunter loves stickers, I will incorporate them as a reward and progress monitor. Once
Hunter masters some alphabet letters and writing he will be given stickers to put on a progress
chart.
