Treatment Plan: Goals and Interventions
Problem 1 (spiritual): feelings of worthlessness, hopelessness, or unforgiveness.
Goal 1: Develop healthy communal relationship with God that leads to
active solutions of current struggles (Kolski et al., 2015).
Objective 1: I need to attend a healthy church on a weekly basis to
meet my needs.
Intervention 1: I will attend Family Worship Center every
Sunday morning to soak up the worship which
ministers to me and the Word of God in order to
advocate self-worth, hope, forgiveness, and joy.
Goal 2: Develop steady prayer and Bible study.
Objective 2: I need to develop a daily prayer and Bible study with
God, further strengthening my relationship with Him.
Intervention 1: I will pray every morning and night, along
with praying before every client. I will enter into a
daily Bible study.
Problem 2 (emotional): long history of depression; severe anxiety; lack of interest
Goal 1: Relieve depressed mood and lack of interest levels and rebound to
prior efficient functioning level (Kolski et al., 2015).
Objective 1: I will complete mental assessment to assess
depression and interest levels (Kolski et al., 2015).
Intervention 1: I will administer the Beck Depression

Inventory-II assessment on a monthly basis to
monitor personal depression and interest levels
(Beck, 1996; see also Kolski et al., 2015).
Goal 2: Relieve anxiety level and rebound to prior efficient functioning
level (Kolski et al., 2015).
Objective 1: I will complete mental assessment to assess anxiety
level (Kolski et al., 2015).
Intervention 1: I will administer the Beck Anxiety
Inventory assessment on a monthly basis to monitor
anxiety levels (Beck, 1993; see also Kolski et al.,
2015).
Goal 3: Develop mental patterns and coping skills to alleviate depression
symptoms (Kolski et al., 2015).
Objective 1: I need to apply behavioral approaches to overpower
depression (Kolski et al., 2015).
Intervention 1: I will coordinate care with Cameron, the
counselor at Baton Rouge General Psychiatry
Department, to establish mental patterns and
alleviate depression problems (Kolski et al., 2015).
Problem 3 (mental): bipolar I; depression; anxiety
Goal 1: Alleviate symptoms of bipolar I, depression, and anxiety.
Objective 1: I need to continue care with my psychiatrist
physician assistant and comply with medication assistance.

Intervention 1: I will continue to see Dominique Anderson
at Baton Rouge General Psychiatry Department as
my psychiatrist physician assistant for mental health
care. I will continue to comply with medication
assistance.
Problem 4 (physical): not enough exercise; overweight, eating improperly
Goal 1: Implement program to get physically healthy.
Objective 1: I need to join a gym to gain physical exercise and to
lose weight.
Intervention 1: I will develop and implement a physical
exercise routine at Snap Fitness 2-3 times per week
as my schedule allows, in order to gain physical
health and weight loss (Kolski et al., 2015).
Goal 2: Implement diet to eat properly
Objective 1: I need to follow a diet that will help me eat properly
and get healthy.
Intervention 1: I will apply the dietician’s directed diet
and
measure my foods so that I can become healthy and
eat properly.
Problem 5 (social): Do not socialize with friends or go places very much; isolation

Goal 1: Socialize with friends more.
Objective 1: I need to get together with friends more often.
Intervention 1: I will get together with friends at least
once
per month and continue to see boyfriend several
times per week. I will continue to call best friends
daily.
Goal 2: Get out more and go places more often.
Objective 1: I need to get more sunshine and go see more places
more often.
Intervention 1: I will make intentions on going outside at
lunch at least 3 times per week and on weekends. I
will make plans to get out of the house and go
shopping, take trips, or just go see a friend for an
hour or so in order not to isolate myself.
