Problems
1. Oppositional defiant disorder- losing temper, irritable, argumentative, defiance,
intentionally upsetting others, blaming others
2. Personal (past history) of neglect in childhood-neglect from au pair, neglect from step-
mother, neglect from father
3. Encounter for mental health services for victim of nonparental child neglect- au pair
bringing other men around, au pair roughly handling the children, not being fed properly
4. Encounter for mental health services for victim of child neglect by parent- father leaving
children with various caregivers while he works away from home for long hours
Goals for Change
1. Oppositional Defiant Disorder
Reduce the intensity of hostile behavior towards others.
Increase consistent behavior and responses to others.
Increase conflict management skills.
Improve communication between client and others that they interact with.
2. Personal (past history) of neglect in childhood
Process past neglect from others.
Increase tolerance for facing adversity.
Increase communication techniques for requesting needs and wants from others.
3. Encounter for mental health services for victim of child neglect by parent
Increase awareness of safety and familial relationships.
Decrease tension between the client and family.
Increase resiliency to cope with family setup.
Therapeutic Interventions
The client will engage in between 3 months to 3 years of weekly, individual sessions of
cognitive behavioral therapy (Aggarwal, 2022).
1. Oppositional defiant disorder
The counselor will provide psychoeducation on the diagnosis and relationship between
hostile behaviors and feelings.
The counselor will assist the client in recognizing and identifying feelings.
The client will implement family sessions to increase communication and reduce
tensions.
The client will practice responses to adverse situations to prepare for decreasing
hostile interactions with others.
The counselor will meet with the client and parents to discuss what behavior

modifications need to be made
The counselor will assist the client and the client's parents in implementing behavior
modifications.
The client will engage in practicing behaviors that they think will decrease hostile
situations.
The counselor will monitor the client's and parent's behavioral modifications.
The client will check in weekly regarding any hostile situations that occurred.
The client will identify and prepare for ways to reduce the risk of relapse.
The counselor will provide booster sessions to ensure the client is continuing to
increase positive behavior.
2. Personal (past history) of neglect in childhood
The counselor will provide psychoeducation on parental relationships and building
trust.
The client will engage in a thought log recording negative thoughts and beliefs about
family structure.
The client will engage in learning techniques to communicate wants and needs to
parental figures.
3. Encounter for mental health services for victim of child neglect by parent
The counselor will provide psychoeducation on safety measures in the home.
The counselor will facilitate a counseling environment where the client can share
feelings about neglect.
The client will engage in psychoeducation on trusting.
The client will engage in conflict management skills.
The client will learn and engage in increasing self-esteem.
Outcome Measures of Change
Improved ability to identify feelings, increase behavior and reaction management, self-esteem,
communication, and trust (Schwitzer & Rubin, 2014).
●The client will report self-awareness of triggers and be able to control emotional
responses.
●Pre-post measures on Connors Child Behavior Checklist and The Child Behaviors
Checklist.
●The counselor observes increased self-control and communication.
●The client reports improved communication with family members.
