Biological
Jeff drinks heavily throughout the week. He drinks an increased amount of alcohol for the
past 18 After two months of his wife’s absence, Jeff experienced difficulties with his sleep
schedule and has experienced months in order to feel more buzzed. His heavy drinking has
resulted in hangovers that are consist throughout the week. Jeff’s hangovers encompass
headaches and feelings of being sick. symptoms of increased fatigue. The client goes to bed
around 2am and wakes up around 2pm, feels symptoms of a hangover, then resumes his drinking
at 5pm until he goes back to bed the following day. Due to Jef’s sad episodes, he also undergoes
a weight loss of 25 to 30 pounds and experiences a lack of appetite.
Psychological
Jeff conveys his depression as “sad episodes” and has escalated due to his marital divorce
and elevated amount of alcohol use. He describes his mood to be predominately sad on a regular
occasion and depicts feelings of hopelessness. After two months of the wife’s absence, Jeff
conveys he experienced a series of suicidal ideations. He researched a plan to kill himself but has
not acted on this plan. Jeff conveys that he has never experienced symptoms of psychosis or
mania.
Social
Throughout Jeff’s adolescence and adulthood, he has attained a friendship with
individuals from high school or work. However, his social life or activities is encompassed by
the use of alcohol. His coworkers have conveyed their concerns of alcohol use and the negative

effect it has on his occupational performance. Jeff is now divorced and has two children. Jeff and
his ex-wife’s relationship are nonexistent due to the three separate acts of domestic violence and
increased use of alcohol that has steadily resulted in a marital divorce. Jeff’s visitation rights are
threatened to become nonexistent due to his elevated alcohol use around his children. Jeff is the
fourth child out of six children and has parents who have been married for a long period of time.
Alcoholism is a common trend in Jeff’s family, as a result, depressive like symptoms are also
prevalent in his family but is never addressed. Jeff also no longer has a relationship with his
younger brother due to alcoholism.

Jeff’s maternal grandparents were raised as Baptists and attended church with them from
