Biological
Jeff’s desire for alcohol has recently increased, and it is taking more drinks for him to feel
the effects of alcohol. Jeff stated when he is at work he struggles with the thoughts of wanting to
drink, has a hard time focusing or thinking about anything else, and his desires to consume alco-
hol results in him leaving work immediately to have a drink. Jeff stated his alcohol tolerance has
increased in the past 18 months. He states he use to be able to drink 4-5 beers and feel the physi-
cal effects, however, now he can drink 5-7 beers and feel nothing at all. On average Jeff states he
drinks about 4-8 beers after work, and on his off days he will drink 6-8 beers a day, plus a few
shots of hard liquor. Jeff is experiencing difficulties physically getting out of bed and to work on
time. He is also experiencing headaches, the feeling of a hangover, and abnormal sleeping habits
and patterns. Jeff states his routine on his off days is to end his night after drinking at 2 am, sleep

til 2pm, start drinking around 5pm, and drink until around 2 am when he is going back to sleep.
He has had 4 violations in the past 18 months, and 2 within the past month concerning his blood
alcohol levels being over the limit allowed to be on his work site. Jeff also mentioned months of
feeling sad, irritable, fatigue, having a low appetite, and having thoughts of suicide, and thoughts
about suicidal plans. Jeff’s loss of appetite resulted in him loosing up to 30 pounds during a sad
time that lasted approximately 7-8 months.
Psychological
Jeff is 33 years old Caucasian male, and recalls feeling sadness since he was 15 years old. Jeff
states he has experienced sadness most of his life, and considers his mother to be a sad woman as
long as he can remember. Jeff states that there has been a change, increase, in the sadness he has
experienced since his separation from his wife. During these periods of sadness, Jeff pulls away
from his family and friends, struggles at work, feels a lack of enjoyment in life, irritability,
fatigue, trouble sleeping, decreased appetite, increased alcohol consumption, and thoughts of
suicide. Jeff was introduced and exposed to alcohol by his father. Jeff grew up in a household
where is father drank daily, and his brother and himself have all share the same drinking habits as
his father. Jeff states his first drink was at the age of 7 with his father and brother, and believe the
act of drinking was something that “men do”. Jeff’s father, 2 brothers, and 2 brother-in-laws all
share similar drinking habits, and Jeff states that drinking is something they do as a family.
Social
Jeff is the 4th child of 6 children, and his mother and father have been married for 45 years. Jeff
has 3 older brothers and 2 younger sisters. Jeff has a good relationship with all of his family
members, except for one if his brothers. Jeff has one brother that has decided to disengage with
the family due to their drinking habits when they are together. Jeff states that when his family is
together, they drink, that’s just what they do. Despite missing his brother, Jeff does not have any

plans on changing his drinking habits to improve the relationship with his brother. Jeff has a
challenging relationship with his ex-wife, and rarely spends time with his 2 children. According
to Jeff’s ex-wife, his drinking was the cause of their divorce. His ex-wife has a hard time trusting
Jeff due to his drinking and the past physical abuse she experience at the hands of Jeff while he
was intoxicated. The last physical altercation resulted in Jeff’s ex-wife needing medical
treatment, law enforcement getting involved, Jeff going to jail, and his wife moving out of there
home while he was in jail. Jeff’s drinking habits is affecting him being able to spend time with
his 2 children. Due to being visibly drunk with his ex-wife picks up the children, his ex-wife is
threatening to have visitation stopped. Jeff is aware that his drinking choices is the reason he
does not see his children often, and he desires to have a better relationship with them. Since
Jeff’s teen years, his friendships consist mostly of individuals who encourage or engage in
similar drinking habits. His high school friends and post high school friends would spend
weekends and off days drinking heavily. His co-workers in his early years would be intoxicated
the night before work and show to work smelling of alcohol. Jeff also states that on his days off
he is almost aways with his friends drinking and getting intoxicated. Jeff’s drinking habits is
affecting the likelihood of him continuing employment at his current job. Jeff has repeatedly
shown up late, violate the blood alcohol screening, and has been suspended multiple time. Jeff
likes his current job, but has recently been threatened with being fired if he does not get
treatment to address his drinking habits. His periods of sadness has affected his work
performance and mood, which his boss has noticed. Jeff stated his family and co-workers have
also commented on the changes in Jeff during his periods of sadness. Jeff is not currently dating
and has no desire to date to get remarried at this time.

Spiritual
Jeff had limited exposure to religion or spirituality. Growing up he would only attend Baptist
church occasionally with his grandparents. Jeff’s family did not and currently do not engage in
religious practices, and Jeff has no desire to incorporate spirituality or religion into his sessions.
