Board to see if she qualifies for upcoming summer mission trips.
Current Situation and Functioning
Ms. O'Reilly is currently leading a highly productive and organized lifestyle. She is
someone who does best with a regular schedule. She has a typical morning routine that
includes getting up at the same time each day and carrying out the same set of predetermined
tasks. After getting ready for the day and eating a healthy breakfast, she always leaves for work
at the same hour. According to the ADLs, Ms. Miranda O'Reilly can care for herself regarding
personal hygiene, nutrition, and elimination (Edemekong et al., 2022).
She always leaves extra time for travel, and she usually gets to work about 30 minutes early
because of the unpredictable nature of traffic. Even though she regularly arrives at work before,
she never wastes time. She also mentioned that she works through lunch infrequently because
it may be used for more productivity. When asked how she handles setbacks, she says, "stressed
out," which describes how she initially feels. She'll take a few deep breaths, redirect her
attention till she's feeling more at ease, and then take a moment to think of a rational plan of
action, which may include seeking assistance from a coworker. When questioned about her
working relationships with others, she claimed they were excellent. Though she finds it more
efficient to operate independently, she values the input of others when considering how to
improve upon a process.

"It felt like everything just became so distorted." She also disclosed that OCD, Generalized
Anxiety Disorder, Post-Traumatic Stress Disorder, and Bipolar type 2 were identified throughout

persistent and annoying. However, she admitted that this inspired and motivated her to resume
her studies. So, she decided to get her Master of Management degree from an online school.
She said maintaining a good GPA while working full-time was challenging, but she achieved it.
She began applying to better-paying corporations and government agencies soon after she
graduated. She said she started getting interviews after using it for approximately five months
and got a job with a significant government agency, where she has remained for the past four
years. Ms. O'Reilly remarked that working for the government differs from working in the
private sector. She said you'll get back roughly the same effort you put in. Her rapid rise through
the government ranks is attributable to her meticulous nature and dogged persistence. She said
she has consistently received high marks for her work, has never been fired or forced to take

She presented herself in a pleasant and friendly manner, was involved in the conversation, and
dressed appropriately for the weather. Eye contact was maintained in an appropriate and
consistent manner by Ms. Miranda O'Reilly. She did not exhibit any abnormal psychomotor
activity and moved in a relaxed manner at the appropriate moments. There were no outward
manifestations of abnormal physical characteristics, and their speech patterns were typical in
that it had a steady flow, a typical tone, and a regular rhythm. Her replies to either receptive or
expressive language fell well within the parameters of what is considered normal. There was no
indication that the individual was now intoxicated with alcohol or substances.
Sensorium and Mental Ability
During the course of the interview, Ms. Miranda O'Reilly demonstrated vigilance and positioned
herself to cover all angles. Although there was not a formal evaluation, Ms. Miranda O'Reilly's
vocabulary and the fact that she maintained a 4.0 grade point average in college both indicate

including abstract thinking, reverse counting, and making associations. It was easy for her to
answer problems that tested her current or distant memory, as well as those that involved

a series of occurrences, we did not see any conceptual thinking difficulties on her part.

I had the impression that Ms. Miranda O'Reilly has a logical way of thinking and that her
line of reasoning was reasonable. Her train of thought was uncomplicated, and she did not
exhibit any signs of grandiosity, irrational reasoning, or loose associations. Her mental process
was within the usual range, and she could correctly recollect the storyline of her favorite film. In
addition, she was able to correctly spell a series of words with six letters both forwards and
backwards. She claimed that her thoughts moved at a typical pace and were not influenced in
any way by outside opinions or observations. There were no instances of thought insertion or

any difficulties with her perception or her aural acuity. All facets of perceptual processing
through the senses, including gustatory, olfactory, and somatic modalities, were demonstrated

action, which may include seeking assistance from a coworker. When questioned about her
working relationships with others, she claimed they were excellent. Though she finds it more
efficient to operate independently, she values the input of others when considering how to
improve upon a process.
Ms. Miranda O'Reilly has found that making other people laugh is a great way to relieve
tension. She views life as precious and finds that laughter is a great way to relieve stress. She
also uses periodic spending on food and other luxuries as a form of self-care. When pressed for
more information, she revealed that once a month, she permits herself to "splurge" on things
like fast food and novelty items she wouldn't otherwise purchase. Many people find Ms.
Miranda O'Reilly emotionally flat, if not flawless. But she also talks about the moments she felt
down and out. When allowed to discuss her depression in-depth, she explained that she
regularly deals with worry and emotions that change "rapidly."
Ms. Miranda O'Reilly has a stellar reputation for her calm demeanor and resourcefulness
under fire. She said that when she's in a tense situation, she can generate multiple mental
solutions to the issue in a couple of minutes. She likes being able to figure out the answers, but
she doesn't like the pressure that comes along with it. Ms. Miranda O'Reilly said that she is
fearless in resolving problems but thinks it's essential to grasp the whole scenario first, whether
in a business or personal context.
Ms. O'Reilly claims that despite keeping her personal and work lives apart, she is still
compassionate. However, she claims that her ability to "feel" people's emotions frequently
makes her uncomfortable and overwhelmed. When questioned about her strengths and
limitations, she thought she was imaginative, sociable, and actively involved. However, she also

mentioned that her inability to remain calm under pressure is one of her most significant
weaknesses.
Relevant Medical History. Ms. Miranda O'Reilly said that her wisdom teeth were
extracted at age 21, and her tonsils were removed when she was 6. Therefore, her family history
of hypertension is confirmed. However, she has never had issues with high blood pressure. Ms.
Miranda O'Reilly was diagnosed with attention deficit hyperactivity disorder during her
childhood. However, she was never given any help for her condition. Three months ago, she
underwent additional testing to confirm the initial diagnosis. She starts her day with 40
milligrams of Adderall. Ms. Miranda O'Reilly also has migraines and takes Topiramate (125 mg)
every night to alleviate the pain.
Psychiatric Treatment History. Ms. Miranda O'Reilly said she only went to therapy once,
and that was to confirm an ADHD diagnosis she'd gotten as a kid from a school counselor.
However, she decided to get help when she started making careless errors at work, kept
forgetting trivial details, and found herself unable to remember even basic knowledge. She said,
"It felt like everything just became so distorted." She also disclosed that OCD, Generalized
Anxiety Disorder, Post-Traumatic Stress Disorder, and Bipolar type 2 were identified throughout
her ADHD evaluation.
The importance of mental wellness was never emphasized in her upbringing. She
speculated that maybe other family members suffer from the same conditions. She also added
that it would be rational, given the way things went down at home while she was growing up.
She used her dad as an example when pressed for clarification. She claimed that his emotions
fluctuated frequently. One minute he'd be OK; the next, he'd be striking her and her siblings
over something minor they didn't comprehend. When asked if alcohol or drug abuse ran in her
family, she answered, "Yes, it runs deep." She revealed that her father is still an alcoholic and
that her mother was a drug addict until she was in middle school. Her great-grandfather and

great-greatgrandfather on her father's side both died of liver cirrhosis when they were in their
thirties and forties.
Family History. Ms. O'Reilly revealed that her father was a cruel old drinker who instilled
a work ethic in her by making her do all the chores around the house. She learned generosity
from her frail mother. She said she had three more siblings who were perpetually at odds with
one another. Despite their frequent bickering, she said she and her siblings were as close as a
"wolfpack." She remarked that the group would rally behind a member in need or danger. She
recalled that a sense of dread dominated her early memories of her father. However, she claims
things are much better and more enlightened now. She admitted that she used to think of her
mother as "flawless" but now recognizes and accepts her imperfections. She also shared that as
her siblings matured, they became closer to her. She remarked that she wished she could spend
more time with them again.
Social and Developmental History
Ms. O'Reilly said her mother was a passive parent, while her father was abusive. She
explained that her family had a tight budget and had to work hard to make ends meet. Because
of that, she was never active in any clubs or organizations outside of the classroom. She said it
was tough to make friends at school because she never dressed well and was probably
hyperactive. She claimed that her peers would mock her because of how she looked, what she
was wearing, or how she smelled. She revealed that she struggled with feelings of inadequacy
throughout her childhood. As an adult, she admits she still has trouble being open and honest
about her feelings.
She elaborated, saying that she doubts whether her friends appreciate her or are just being
polite.

Ms. Miranda O'Reilly said she moved about a lot and had a bad mother but did well in
school despite this. She admitted that her enthusiasm for education was widespread. However,
she said that everything was delivered rapidly to make her bored. She claimed that she
frequently found herself struggling to fulfill her responsibilities. When asked about her
interactions with authority figures, she admitted that she used to be afraid of them but has
since come to view them less as an enemy and more as a task to be overcome. She also said
that as she got older, her teachers began challenging her with more complex assignments or
letting her assist them when they saw her grasping topics more quickly than her classmates. Ms.
O'Reilly has been in more than one serious relationship. However, she said it was hard for her to
become serious because she worried about being "controlled" by a male.
Educational and Occupational History
Ms. O'Reilly remarked that she felt "lucky." She received a full academic scholarship to a
prestigious institution, where she studied Business Administration because of her outstanding
performance in high school. She was proud of her first-degree performance, she said. But she
lamented that she had yet to do better. She admitted that she squandered much of her
"freedom" by partying instead of helping others. After finishing college, she got work in a
healthcare facility, where she would pursue payment from people who owed money. Ms.
O'Reilly said she "hated" her position because she made people who could not pay their bills
even worse. She said that eight months into working there, she finally had enough. She felt so
bad that she quit and looked for a lower-paying position where she wouldn't have to make
collection calls. She quickly got another office work, but this time the sexual harassment was
persistent and annoying. However, she admitted that this inspired and motivated her to resume
her studies. So, she decided to get her Master of Management degree from an online school.
She said maintaining a good GPA while working full-time was challenging, but she achieved it.

She began applying to better-paying corporations and government agencies soon after she
graduated. She said she started getting interviews after using it for approximately five months
and got a job with a significant government agency, where she has remained for the past four
years. Ms. O'Reilly remarked that working for the government differs from working in the
private sector. She said you'll get back roughly the same effort you put in. Her rapid rise through
the government ranks is attributable to her meticulous nature and dogged persistence. She said
she has consistently received high marks for her work, has never been fired or forced to take
time off, and has never been hurt on the job.
Cultural Influences
Ms. Miranda O'Reilly, who grew up in a low-income area, considers herself fortunate to
have experienced such a multicultural upbringing. She reflected on her childhood, noting that
she was often self-conscious about the fact that her skin was noticeably paler than that of her
peers.
She did, however, admit that she thought that to be fascinating. She also mentioned that the
opportunity to try new cuisines was one of the best parts of living in a multicultural community.
Mental Status Exam
Appearance and Behavior
Ms. Miranda O'Reilly reported that she was 5 feet 7 inches tall and weighed about 130 lbs on
average. She described herself as a white woman who was unmarried and appeared to have a
rosy complexion and peach-colored skin. Her appearance was impeccable; her hair was sleek
and straight, and her clothes were ironed and made to fit her perfectly. When walking across
the room and back to her seat, Ms. Miranda O'Reilly appeared to have an average gate and

motor coordination. As a result of the emergency surgery she required after her vehicle
accident, Ms. Miranda O'Reilly developed a scar below her right eye and beneath her eyelid.
She presented herself in a pleasant and friendly manner, was involved in the conversation, and
dressed appropriately for the weather. Eye contact was maintained in an appropriate and
consistent manner by Ms. Miranda O'Reilly. She did not exhibit any abnormal psychomotor
activity and moved in a relaxed manner at the appropriate moments. There were no outward
manifestations of abnormal physical characteristics, and their speech patterns were typical in
that it had a steady flow, a typical tone, and a regular rhythm. Her replies to either receptive or
expressive language fell well within the parameters of what is considered normal. There was no
indication that the individual was now intoxicated with alcohol or substances.
Sensorium and Mental Ability
During the course of the interview, Ms. Miranda O'Reilly demonstrated vigilance and positioned
herself to cover all angles. Although there was not a formal evaluation, Ms. Miranda O'Reilly's
vocabulary and the fact that she maintained a 4.0 grade point average in college both indicate
that she has an IQ that is above average. Ms. Miranda O'Reilly was challenged with activities
including abstract thinking, reverse counting, and making associations. It was easy for her to
answer problems that tested her current or distant memory, as well as those that involved
mathematical computations. In addition, when we asked her to explain the differences between
a series of occurrences, we did not see any conceptual thinking difficulties on her part.
Thought
I had the impression that Ms. Miranda O'Reilly has a logical way of thinking and that her
line of reasoning was reasonable. Her train of thought was uncomplicated, and she did not
exhibit any signs of grandiosity, irrational reasoning, or loose associations. Her mental process
was within the usual range, and she could correctly recollect the storyline of her favorite film. In

addition, she was able to correctly spell a series of words with six letters both forwards and
backwards. She claimed that her thoughts moved at a typical pace and were not influenced in
any way by outside opinions or observations. There were no instances of thought insertion or
broadcasting that could be found. There was no indication of paranoid ideation, ruminations,
hallucinations, compulsions, or obsessions that was displayed or present.
Sensory Motor and Perceptual Processes
During the examination, both the perceptual process and the sensory motor skills were found to
be within normal norms. Ms. Miranda O'Reilly was able to faithfully reproduce contour
drawings and a straightforward portrait without exhibiting any trembling. She did not exhibit
any difficulties with her perception or her aural acuity. All facets of perceptual processing
through the senses, including gustatory, olfactory, and somatic modalities, were demonstrated
to be operating within normal parameters. Ms. Miranda O'Reilly had no trouble concentrating
and staying on target with the assignments that were given to her.
Affect and Mood
During the course of the interview, Ms. Miranda O'Reilly was cheerful and did not exhibit
any signs of pain or a downcast disposition. Her facial expressions suggested that she was
smiling all the time, and she expressed the proper emotions as the test went on. If you were to
ask Ms. Miranda O'Reilly about her feelings at the moment, she would tell you that she is
pleased and happy. The reason why she would say this is because she recently had dinner with
her friend. Her demeanor remained unchanged during the entirety of the evaluation, and she
maintained that she was feeling pleased in general. When confronted with embarrassing and
insulting questions, there was no discernible change in disposition or effect. She has no history

of manic-like symptoms, suicidal thoughts, or homicidal ideation, nor does she have a family
history of either. There was no evidence presented of any potential for aggression or impulsivity.
Self-regulation
Ms. Miranda O'Reilly exhibited typical levels of judgment and the ability to regulate her
impulses. She provided an accurate explanation of self-regulation by means of talking,
monitoring, establishing goals, and reinforcing positive behavior. Ms. Miranda O'Reilly was able
to comprehend her acts and how those actions affected her, as well as keep track of how she
responded to the people in her immediate environment. She provided a correct explanation of a
self-regulation theory when she was presented with a simulated scenario. According to this
theory, an individual's environment, behavior, and self all need to be taken into consideration
for one to successfully self-regulate. These attributes that she displayed in the interview are
congruent with her past.
