A. Safety Programs
Dedicated, professional pilots and mechanics, good management, excellent
equipment, standards that are set and adhered to, and perhaps, most importantly, a
corporate and departmental culture of conservatism are the causal factors in a safe
operation. This last item is most important because safety as a concept and safe

While it is possible to run a safe flight department without a high-level commitment to
safety in the organization, this is a rarity and a credit to the professionalism of the
individuals within the department.
The company purchased its aircraft to realize the benefits of on-demand air
transportation: to go when and where its personnel want to go, do it expeditiously, and,
most importantly, do it safely. At times, the when, where, and expeditious qualities may
be in conflict. The boss wants to go to his lakeside home in northern Michigan in
February for a hunting trip with his friends. The narrow, 4000-foot, single strip serving
the destination has only a VOR approach to 600-2 minimums, and it is often unattended
during the winter. At departure time, light snow and strong crosswinds are forecast for
the destination; arrival will be just after dark with the prospect of a circling approach to
minimums. The nearest decent alternate is 100 kilometers (km) away, a 2.5-hour drive
away from the boss’s house; and fuel could be tight on a divert. It is clear that the boss
wants to get there as soon as possible to get the weekend started.
In the table, it is gratifying to note that, over the most recent five-year period of
available data, the business and corporate aviation accident rates have continued to be
relatively stable and compare favorably with airline mishap rates. The corporate rates are
a testament to the care and management of those operations from both the flight
department and the companies that own and operate the aircraft. Yet we are still plagued
with the fact that more than two-thirds of business aircraft accidents occur in the final
minutes of the flight, i.e., the approach and landing phases. Similarly, another 15 percent
of accidents happen during the takeoff and climb phases, which take up approximately
three percent of flight time. While it may seem logical that many accidents would occur
during the time the aircraft is closest to the ground, concentrated focus on these
percentages has yielded little change in the rate of occurrences. These are clearly areas
that we must work on if improvements in the overall accident record are to be made.
Additionally, non-flight related accidents are often overlooked in a safety
overview of business aviation operations. These occur mostly to individuals in
maintenance, fueling, aircraft ground movements, and hazardous materials handling.
While much emphasis is placed on protecting aircraft, passengers, and crew in the flight
regime, less attention may be focused on ground operations, which often feature very
damaging and debilitating personnel injuries. Many of these mishaps are also classified
as incidents, not qualifying for either the ICAO or US NTSB definitions for accidents;
but incidents can often be serious to the point of being as bad as an accident.
Hangar safety is for the technicians, right? So, the pilots can skip this part? Well,
no…the number of pilots injured in industrial-type accidents often exceed the number of
technicians injured. The next most likely personnel to be injured are passengers passing
through the hangar en route to their aircraft. So there are inherent hazards within the
confines of the hangar. But awareness of these potential problems may be a greater
problem than the existence of the hazard. No one wants to think about injuries, hazards,
and the like. Everybody would rather think about the task at hand, to be goal directed and
operationally minded. If your company has a company-wide safety program, there are
probably scheduled inspections conducted and/or checklists available to help with this
essential safety task. If your company does not have any of this available, you can
conduct your own safety inspection using readily available checklists and reference
documents.
B. Safety Culture
Over the past 50 years, organizations involved in critical operations, such as
nuclear power, human surgery, and oil exploration and production, have discovered that
merely having a series of unrelated and poorly coordinated processes and procedures do
not make an effective safety program. Rather, a series of mature, coordinated, and fully
integrated measures all designed to create a risk-averse environment provides superior
safety within complex organizations. During the past half-century, the previously
mentioned fields of specialization and aviation interests have developed a series of
policies, processes, and procedures known as a safety management system (SMS) to
provide a positive safety environment. Much of this chapter will be devoted to a
discussion of these systems and how they operate. But first, before any consideration of
safety systems can be made there must be a foundation for SMS implementation.
Each organization develops a distinct set of behaviors over time that gives it a
unique identity. This organizational culture consists of its values, visions, norms, working
language, systems, and symbols; it includes beliefs and habits. It is also the pattern of
these collective behaviors and assumptions that are taught to new organizational
members as a way of perceiving, and even thinking and feeling. Organizational culture
affects the way people and groups interact with each other, and with others outside the
group. Organizational culture is an important aspect of any organization, since it provides
the groups identity and justification for existence. Safety is an important component of
culture, since it encompasses a number of organizational norms including risk perception,
adherence to standards and procedures, interaction with others in critical situations, a
cautious view of goal direction and respect for other team members opinions, and views
of critical situations.
Dr. Reason describes the just culture as an atmosphere of trust in which people
are encouraged, even rewarded, for providing essential safety-related information, but in
which they are also clear about where the line must be drawn between acceptable and
unacceptable behavior. So it’s just not a free ride or a get-out-of-jail-free card when an
individual crosses the line between behavior that is good, reasonable, and acceptable and
that which is irresponsible, contrary to accepted procedures, and potentially damaging.
Just culture refers to a set of organizational behaviors that promotes a questioning
attitude, is resistant to complacency, is committed to excellence, and fosters both
personal accountability and corporate self-regulation in safety matters. This series of
policies and behaviors is attitudinal as well as structural and relates to both individuals
and organizations. Personal attitudes and corporate style can enable or facilitate the
unsafe acts and conditions that are the precursors to accidents and incidents. A just
culture requires not only actively identifying safety issues, but also responding with
appropriate action.
Most of all, the just culture centers and is based on total mutual trust within the
organization. Without trust, there can be no just culture and, unfortunately, no real safety
culture. The non-trustful organization will be reduced to safety slogans, empty
pronouncements, nonstandard behaviors, and a reluctance to become truly risk averse. I
hate to make it sound so grim and unremitting, but without trust at all levels, the
organization reverts to a series of individuals marching to their inner safety drummer.
C. Managing Safety
Most pilots and mechanics who have been in the business for a while are familiar
with safety programs. In fact, they are familiar with a variety of programs from the past
that were tried and discarded with such regularity that they were known as the “program
of the week.” This was the case because most people and organizations focused on hit-or-
miss accident prevention and causes rather than on a comprehensive, fully integrated set
of policies, processes, and standards. All efforts were focused on foreseeing the most
common accident types and building barriers to prevent their occurrence. In reality,
accidents never are caused by a single failure such as lack of training, poor judgment, or
inadequate supervision; accidents are caused by lack of adequate policies, failure of
management to enforce existing standards and norms, poor supervision, a desire to
accomplish assigned duties and missions without the benefit of assessing the attendant
risks, and inadequate operational policies and standards.
This piecemeal and fragmented approach was only marginally effective because it
neglected the need to address proximate and underlying causes of an accident and to
achieve an all-encompassing view of the entire operation. Most operators believed that
with a little bit of training, a few policies, some standard operating procedures (SOPs),
and occasional reviews of selected operational risks all would be OK. Yet, these items
were not well balanced or coordinated—the trainers did their thing, policy makers
worked in an organizational vacuum, SOP designers did what they thought best, and
management tried to keep up with it all. What was really required was a comprehensive,
flight-level 410 view of the entire operation and its environment. Moreover, all of this
needed to be tied together with well-integrated policies, processes, procedures, personnel
accountabilities, and a feedback mechanism to tell all concerned whether the organization
was meeting the goals it had devised for evaluating the entire safety picture of the
operation. Enter the SMS.
Over the past 20 years, a systems approach to safety emerged from several
industries and activities involving critical operational elements. Among these are nuclear
power, medical surgery, and oil and gas exploration and production. Military aviation and
the airlines were the first to adopt the principles laid down by the original critical
occupational specialties. Most corporate and charter operators are aware of the existence
of SMS, but not all may fully appreciate the rationale for its use and the advantages it
brings to an operator. They practice some form of overall safety program, especially if
they have an up-to-date flight operations manual that contains policies, standards, and
procedures that apply directly to their operation. What is missing is the integration of
these elements into a system of interlocking policies and procedures that considers all
elements in concert.
More importantly, the SMS forces an operator to actively consider potential

organizational and environmental change, internal evaluation, and program revision.
Now that ICAO has made it mandatory for large and turbojet-powered aircraft
operating internationally to “establish and maintain a safety management system (SMS)
appropriate to the size and complexity of the operation” after November 18, 2010,
implementation of the system is in full swing. Experience has shown that both grasping
the SMS concept and implementing it are not particularly easy tasks for corporate
operators. The fully integrated approach to safety, and the diversity of missions and wide
range of flight department sizes make a one-size-fits-all approach to SMS impossible.
