Health Behavior Change for wellness

Robin Majeski, PhD, RN, Clinical Associate Professor, The Erickson School at UMBC

Learning Objectives

Describe elements of the transtheoretical model of change and health belief model.

Demonstrate how you would apply the transtheoretical model of change and the health belief model to foster health in older adults.

Analyze the advantages and disadvantages of the transtheoretical model of change and the health belief model

Health Behavior Theories/Models

Description of factors which impact health behavior change, especially motivation for change (e.g. motivation for eating a healthier diet, engaging in healthy exercise regularly) and the process through which health behavior may change in a positive direction.

Health Belief Model: General Points*

*Health Belief Model (n.d.). https://healthbeliefmodel.weebly.com/terms-and-key-points.html

6

Importance of individual perception of susceptibility to illness

Importance of individual perception of effectiveness of treatment or health behavior change in alleviating vulnerability to illness.

Health Belief Model: Key Factors Which Motivate Health Behavior*

Factor Example
Perceived susceptibility to illness Perception of one’s susceptibility to heart disease
Perceived severity Perception of the severity of heart disease (e.g. is it seen as potentially life-threatening?)
Perceived benefits Perception of the benefits of engaging in more exercise with reducing risk of heart disease.

7

Health Belief Model: Key Factors Which Motivate Health Behavior*

Factor Example
Cues to action Ways of promoting readiness for engaging in more exercise to reduce risk of heart disease. This could be working with a health coach who supports change in the person.
Self efficacy Confidence in one’s ability to take action and engage in more exercise to reduce risk of heart disease.

Health Belief Model: Modifying Factors*

Modifying factors affect the key factors

which motivate health behavior

Education

Culture

Age

Personal experiences

Gender

Socioeconomic status

9

Transtheoretical Model*

Stage Description/Example
Precontemplation Not even considering changing behavior, e.g. with diet, exercise, alcohol use
Contemplation Beginning to consider one may have a problem with lack of exercise, poor diet, excessive alcohol use.
Preparation/Commitment Deciding and planning change

*Gold, M. (2018). Stages of change. https://psychcentral.com/lib/stages-of-change/

10

Transtheoretical Model*

Stage Description/Example
Action Taking steps to change diet by eating healthier foods, beginning to exercise more regularly.
Maintenance, Relapse, Recycling Diet changes are established may revert to earlier less healthy patterns but returns to health diet quickly.
Termination Unhealthy diet is no longer a threat, few if any relapses to unhealthy diet,

Apply the Models!

Identify a health concern you have or someone whom you know may have. This could include high blood pressure, STDs, diet, exercise, pregnancy prevention, injury prevention, or other health concern.

Identify a health behavior you or someone whom you know may need or want to change (e.g diet, exercise, using protection against STDs, etc).

Apply the Models!

Identify the stage of change you or the person whom you know may be in with this health behavior. Also, what strategies might help you or this person to complete this stage and move to the next stage of change?

Apply the Models!

What strategies might move you or the other person to the next stage of change? What might motivate you or the other person to change health behavior?

Using the health belief model, identify components which affect your health behavior or the health behavior of someone whom you know. How do these affect the likelihood of you or the other person engaging in healthy behavior with the selected health concern?
