What Measures Could Prevent Such a Crisis?

A patient in your hospital was placed in restraints due to agitation and to prevent her from harming herself or others. During normal morning rounds, a nurse discovered the patient dead in her bed. As the administrator, how do you handle this crisis? And what measures could you put in place to prevent something like this happening again in the future?
