a questionnaire design and one page illustration of the questionnaire

The due time is 12 pm tomorrow (04/04/2016) New York time zone

i need u to design a questionnaire for my program and about 15 questions with multiple choice and few open-ended questions.

Also, u need to write several paragraphs about:

1.    

3.    Write a paragraph about how you will determine the validity and reliability of your instrument, including any pilot testing that will occur.

4.    Write a paragraph about who will complete the instrument and any sampling strategies that will occur.

My program is in appendix.  also the requirement of questionnaire is in appendix ( but it is just for reference)
