Project Proposal outline/references/information for Sight Center/Visually Impaired Community

I have a project proposal that I need to create that will be considered a needs analysis within the Cleveland Sight Center. This will be a proposal to assist with the community's needs for visually impaired people, diabetic retinapathy, and how a program can be developed to support this community. Create one policy that can support the community of low income population that suffer with developmental or onset/sudden vision loss, due to diabetes. Public information/resources can be referenced within the proposal.
