Instructional Unit Lesson Plan in Elementary Education

This is a unit lesson plan and requires 5 separate lessons combined into this one lesson.  I have attached all of the information that is required.  I want a plan that has never been used before.  I have already had this happen to me and can not afford it again.  I am up for any grade level from 1st-6th.  I am in a Math teaching class so it needs to be tied in with math somehow.  I need this by Friday August 29, 2014.
