The advanced directive project is to get you thinking deeper about what you or a loved one believes is the best way to experience the dying process

For this project you should use the 5 wishes form provided to you. Complete this form either for yourself or interview a loved one to complete the form for him/her.

Once the form is complete write a brief reflective paper on what you learned through this experience. Include if you discovered anything new or changed your mind about anything through the process.

This paper should be at least 2 pages double spaced but no longer than 5 pages double spaced.

Only this reflective paper needs to be turned in. 

This assignment allows you to connect more with your feelings dying and the dying process.
