Critical Clinical Issues

This module’s intent is to discuss and make correlations between mental health issues and involvement with the legal system. One of the critical clinical issues to explore is the issue of abuse and its effects on mental health. Child abuse is a major public health issue and can manifest in many ways. Child abuse or maltreatment includes neglect and physical, verbal, sexual, and emotional abuse. Recent research indicates that about 3.5 million children are reported for suspected maltreatment in the United States each year (Strathearn et al., 2020). The short-term and long-term effects of child abuse on children are complex and diverse. Long-term effects associated with maltreatment include cognitive disability, anxiety and depression, psychosis, teen-aged pregnancy, addiction disorders, obesity, and cardiovascular disease. Childhood sexual abuse has an increased risk for psychopathology and suicidality, risky behaviors, physical illness, education and employment challenges, and crime and violence. Understanding the various ways a child could be mistreated and potential impacts on their well-being and treatment could help increase the chances of a better prognosis. Each case is different, and the signs of neglect, abuse, or maltreatment may vary.

General environmental factors that may indicate neglect or a form of abuse include the following:

  • Impaired parenting ability
  • Low SES
  • Negative view of an infant, child, or adolescent
  • Inadequate healthcare for the child
  • Lack of education
  • Pregnancy complications
  • Impairment of the parent-child relationship because of mental illness
  • Increased intrafamilial stress
  • Parents with psychiatric disorders
  • History of childhood trauma in parent
