The operators is caught-in the heavy trailer which is likely to tip. He can be pinned between the heavy trailer
and the chassis of the truck. The injuries can that can happen is bones breakage, suffocation and death. The failure of the
hydraulic system can cause the heavy load to tip .When the hydraulic bed is being lowered the person is in danger of being caught-in between

What would you recommend to correct the hazard?
Do not operate between unsupported hydraulic system
Ensure after the support, the machine is de-energized to prevent unintentional start
When operating under heavy load give extra support to the hydraulic system to prevent falling
Do not work or carry out inspection in between dump box and the frame of a dump track

The caught-in between in this case happens due the opened or exposed moving parts. Placing hand or operating the machine
while the moving parts is exposed is a hazards. When the machine is not fitted with lock and tag out system they can start automatically and
expose the operator to injuries. The system has inadequate guards thus increase the hazard level

Use machines which are properly guarded
Do wear loose clothes when operating moving machines

The worker in the trench is exposed to hazards of potential cave. He is also caught-In between the unprotected sides of the trenches.
The materials overhead expose the person working in the trench to falling objects. The operation or working near a structure is dangerous as it

What would you recommend to correct the hazard?
According to OSHA, workers inside trenches or excavation should be protected from potential cave-in
In this trench there are no adequate slope or shore
There is no protective system in this trench which
According to OSHA the maximum travel distance to the ladder position should be 25 feet's
When working in a trench, the side who have a slope to ensure the wall does not collapse. This angle depends on the type of soil.
Use trench box, this does not prevent the collapse but it protects the workers working in the trench when cave-in occurs.
The workers should construction shoring to prevent the wall from collapsing,
Do not work in trenches ensure all protective systems are in place
The employers should ensure there is easy way out of the trench
When working in confined space, there should be adequate instruction to ensure safety

The hip of soil block the visibility of the pit further exposing machine and people to the risk of fall

What would you recommend to correct the hazard(s)?
The site should be barricaded
Heavy machines should operate near this trench, they can cause cave-in or possibly fall in the excavations
The pit should be inspect and the type of soil determined to ascertain degree of collapse.
Trained personal should identify the potential hazards before operations.
When operating in open pit, there is need to barricading
Heavy vehicle should come near the edge of the trench or pit
OSHA rules demand inspection of opening, trenches and pit before proceeding with work
