Module 5 ENCOMP AND SBAR
NURS 3315 Holistic Health Assessment
Patient Name: John Doe/Jane Doe
Student Name_________________________ Date____________________
Module 5 assignment
(Ecomap and a final SBAR are the required assignments for this module.) The final SBAR must be covering a new problem, different from ones you have covered in previous weeks. It needs to be concerning an individual person in the patient role, and not for instance, a unit, a coworker situation, or piece of equipment. Submit the SBAR on a topic that could occur in the patient population in which you work. If you are not currently working, complete one on a situation you have observed to demonstrate your ability to apply the SBAR concept to a situation. (It can be about an infant, for instance, if you are a NICU nurse.)
Final SBAR

SBAR |
Situation (What is a problem you could encounter in the course of your job? Compose a scenario to answer the following: When did it start, and how severe is it?)
Background (The evidence—Health history relating to this problem, what is being done, and what assessment findings are most important now.) |
Assessment—Name the problem or the direction things may be going (This is not the place for nursing assessment findings—those go under Background—see the Module 1 Kaiser documents.) If the quickest way to name the situation is use a medical diagnosis, that is allowed in an SBAR to facilitate communication—in that state, “It may be ____”. |
