Tips for Creating Your Customer Service Survey in SurveyMonkey

Following are some tips for creating your customer service survey for your course project assignment:

1. Sign up: Sign up for a new account with SurveyMonkey at http://www.surveymonkey.com. SurveyMonkey will start building your profile. When you are prompted to specify an organization name, type, and industry, provide a fictitious name for your organization, select Individual as the organization type, and select Education as the industry.

2. Create your survey: Click the Create Survey button. Be sure to build a new survey from scratch. Do not start from a sample healthcare or customer service template provided by SurveyMonkey. (SurveyMonkey will refer to this option as something like "Build a New Survey from Scratch" or "Write Your Own Survey Questions".) Your instructor has access to the SurveyMonkey templates and will be able to detect whether you wrote unique questions or copied them from a template.

3. Give your survey a title: Type a name for your survey (for example, "Patient Satisfaction"). When you are prompted to select a category, select Other.

4. Add survey questions: On the left side of the Design Survey screen, you will see a Builder section similar to the following: NOTE: Do not use the Question Bank section beneath the Builder section.

Click the type of question you want to add (for example, Multiple Choice) and drag it to the work space in the middle of the page. Be creative and explore all of the options! Clear the Show Suggested Questions check box so it looks like this: Type your question and answer choices, and then click Save when you are done. Continue adding questions in this manner.

5. See what your survey will look like to your users: Click the Preview and Test button. Click Cancel to exit the preview.

6. Create the web link for your assignment: Click the Next button. SurveyMonkey displays a screen that looks similar to this: The part in red is the web link you will need to provide to your instructor for this assignment. Copy the web link and paste it into the assignment drop box.
