Performance objective

You must gather feedback from a supervisor and a colleague on your personal work performance and use it to identify areas for improvement and ways it can be achieved.

Assessment description

You will need to use the form included in Appendix 1 to assess yourself and gather feedback from others on your personal work performance.

You will then need to arrange a performance appraisal with a supervisor and review the feedback to identify ways in which your work performance could be improved to meet team and organisational standards.

You will then need to discuss and agree on ways you can improve your work performance by adjusting your current work and identifying and planning to undertake opportunities for improvement.

If you do not currently work for an organisation in which this task can be undertaken, discuss and agree on a suitable alternative with your supervisor.

Procedure

1. Appendix 1 contains three copies of an assessment form.

a. This form will be used to assess your ability in the various tasks in your work role. Marks from 1 – 10 (with 1 = poor and 10 = excellent) should be entered against the tasks in the left hand column to grade your ability.

b. You will need to complete a copy of the form yourself and ask a supervisor and a colleague to also complete copies, grading your abilities. Ensure that they record their name, position and sign and date the form.

c. If a task on the form is not part of your role, mark it ‘n/a’. If there are common tasks that you undertake in your role that are not on the form, enter them into the blank spaces at the end.

2. Arrange an appointment with your supervisor at which you can discuss the feedback you have received. During your appointment, liaise with your supervisor to gather the information needed to complete the tasks in Appendix 2.

Specifications

· You must gather feedback and undertake your performance review as soon as they are able to be organised.

· You must submit your self-assessment form, along with those of your colleague and your supervisor. These must all be complete and have signatures and dates.

· You must submit your assessment task recording the findings of your performance review so that they can be clearly read and understood by your assessor.
