SECTION TWO: Proposal Outline - Final Project Initiative Proposal
Write 2-3 paragraphs comparing and analyzing your city or county data with US data (or your country).
Looking at the data between the two there is a lot of similarities. Life expectancy are within in a few years of each other, as well as income levels, poverty levels within the county level and national level you can see that there is a problem. The top 10 causes of death within Harnett County and the US, are all about the same the only difference is that they are in different orders. Suicide is on the top 10 causes of death for both locations.
State the health issue you are choosing for your population health initiative (one – two sentences): The population health initiative that I am choosing for my project is Suicide. Mental Health is something that has become a taboo and I want to do more research on how this correlates with suicide.
Explain why you chose this health issue (one - two paragraphs): I have chosen suicide because it’s an issue that hits close to home. Suicide is a major public health concern. Suicide doesn’t discriminate against, sex, age, gender or even ethnicity. Over 41,000 people within in the USA lose their lives to suicide.
Describe your chosen population. Be specific by providing demographics to fully explain your population
(provide two – three sentences to explain why these demographics are important): With a public health approach,
prevention occurs at all levels of society—from the individual, family, and community levels to the broader social
environment. Effective prevention strategies are needed to promote awareness of suicide while also promoting
prevention, resilience, and a commitment to social change.
List and describe three objectives you hope to accomplish with your initiative (one sentence for each objective).
Use the SMART topics as a guide. (SMART – specific, measureable, actionable, reasonable, time bound)
1. Learn the warning signs to help prevent suicide.
2. Increase the proportion of primary care facilities to treat mental health.
3. Break the shame and taboo on suicide.
Describe two – three activities you will use to accomplish each of these objectives (one – two sentences for each activity).
