I provided an example of how the ppt is suppose to be however using THIS case

Demographics

  • 87yo Caucasian male who resides in Angela Hospice Center
  • Asked his dental office if he could have his teeth cleaned before he died.
  • RDH is sent to the center to treat the patient.

Medical History

  • History of severe cardiac problems and pulmonary disease.
  • High blood pressure and difficulty breathing.
  • He also has a very constricted aortic valve.
  • As a patient of a hospice center he is not being treated for his blood pressure or pulmonary problems but is being kept comfortable.

Medications

  • Ativan
  • Roxanol™ (Morphine Sulfate 20 mg/mL).
  • Past history of multiple blood pressure and respiratory medications but is no longer taking them.

Dental History

  • The patient has not been a regular patient at his dental office.
  • He has not been to the office for 5 years except a visual exam since he has been a resident of hospice.
  • His missing teeth are. #1, 3, 4, 5,12, 13, 14, 16, 18, 19, 21, 30 and 32.
  • He has a full gold bridge on #30-31 with an occlusal rest on fractured and decayed #29.
  • Tooth #17 is horizontally verted and coated in calculus

Periodontal History

  • The patient had a high plaque level and OHI was poor.
  • He has 4-5mm pockets on all posterior teeth
  • Tissue is erythematic
  • Moderate bleeding when his teeth are brushed.

Periodontal Charting

  • No charting done. The patient is in the active phase of dying with no good prognosis of oral healing.
  • Visual diagnosis indicates severe periodontitis.
