Proposed Action Plan

Action plan on helping women prevent heart disease.

Mission statement

Our mission statement is to enhance the standards of living in terms of health among the women that have been diagnosed with the heart diseases and those who have not. This mission will help serve all the women to ensure they have both benefited from the programme. Women will be educated to ensure that they have the basic knowledge required concerning the condition and testing will be made available so as each one f them has an idea of her condition (Moodie, 2016).

Vision statement

These conditions are highly preventable therefore all women will be free if necessary information is passed to them and prevention programs initiated so as to help prevent future possible conditions. The vision will help generate a strong foundation programme that will help both young and aged women through education and provision of necessary services to those with heart disease.

Objective

The objective of this program is to ensure that the numbers of women with heart disease are reduced to the minimum level possible within the set period of time. Active involvement by the women is necessary in health programs. The program hopes to ensure that all the women have access to medical facilities and proper services provided to ensure their conditions are improved (Regis, 2013).

Strategies

The program will be conducted through a campaign that will give all women an opportunity to take part. In additional seminars will be conducted in all the regions of the country to ensure that all the women have received information. After the campaign and seminars a free testing will be conducted to have all the women screened.

Action plan

In order to prevent the heart diseases every woman is required to take the several measures including deciding on a healthy eating plan. The food people eat can reduce the danger of heart disease for example low saturated fat food and a lot of fruits, physically active for example walking few miles jogging among others. Furthermore the medical healthcare will be added more infrastructures so as to cure those with the disease.

Action steps

Some of the members of the team accompanied by professional doctors will take part in conducting visits to various residents to check on their diets and educate them on the correct diet required. Each lady is required to submit the progress forms at the regional hospital and pick another one so as to have a follow-up on each and every person. This practise will go on for a period of six months. The funds to support the program are available where we received donations from various institutions including the government and non-government organisations. Each and everyone citizen in the country should know about this information.

Implementation of action plan steps

First the door-to-door visits will be conducted first since it’s the easiest way of communicating. Later the progress follow up from the forms filled will be conducted and after period of six on the screening will be conducted to evaluate the results and compare it with the earlier results to find out any progress from the program.
