Article 1.....Regulations for Long Term Care Facilities.
A. Identify by name and location and research a Long Term Care Facility that had a regulatory deficiency. Kids Behavioral Health, Montana
-What was the deficiency? It was found by another incident which involved an assault of a minor who was staying there for psychological issues. During the investigation it was found that the organization did not increase staff numbers to properly monitor and treat recently admitted patients with aggressive behaviors leaving other patients vulnerable. The environment also led to the inappropriate restraint and seclusion of many youth. That violates §483.364- Monitoring of the resident in and immediately after seclusion
-How was the deficiency addressed by the facility? Their was an investigation by the Montana Advocacy Program, Department of Health. After the investigation was complete, a list of recommendations were made, KBH was ordered to change some of its policies.
Were there any penalties involved? I could not find any specific penalties such as monetary or job loss.
Article 2, CON
A. From the map choose a state that has CON regulations. Illinois
B. From that state, identify a hospital/ health system that had project review by CON. MercyRockford Health System
C. Describe the project and the outcome of the CON process. MercyRockford Health System plans to create one hospital on two campuses in Rockford, enhancing access and health care services for the Rockford community. It is still in review by the board and if approved, construction is expected to start later this year.
Article 3, Accreditation,
A. Joint Commission on the Accreditation of Healthcare Organizations (JCAHO)....define their mission/role. Their mission is "to continuously improve health care for the public, in collaboration with other stakeholders, by evaluating health care organizations and inspiring them to excel in providing safe and effective care of the highest quality and value".
B. Identify and describe one accreditation standard for hospitals. One accreditation standard for hospitals is Rights and Responsibilities of the Individual. This standard addresses informing patients of their rights, fostering an environment where a patient's values, beliefs and preferences are respected. Also, keeping the patient involved with their progression of treatment as far as making medical decisions.
C. Identify a hospital by name/location that has JCAHO Accreditation. From your research, describe the published results of their last survey. Massachusetts General Hospital located in Massachusetts, has JCAHO Accreditation. They were surveyed April 13 to April 17, 2015. Some areas that needed improvement were records, improvement on infection control, and a few recommendations in safety. Overall the hospital had a good rating.

James,
I believe it is important to have CON laws in place to help keep healthcare costs low. If we continue to allow a large number of facilities to open up and they are unable to cover their costs due to a lower number of patients then the cost of healthcare will increase. Health care is expensive enough already. Where I live in Charles County, MD there is one small hospital and recently three urgent care facilities have opened up. I assume that these urgent care facilities have been opened to help in reducing the number of non-emergent visits to the hospital. This way the hospital is able to focus on more critical issues. Urgent care facilities also help those patients who are unable to get an appointment with their primary care physician.

Querencia at Barton Creek – Austin, Texas
-What was the deficiency?
January 08 2015 during the most recent inspection there were a total of four deficiencies noted: two health, one environmental, and one pharmacy.
-How was the deficiency addressed by the facility?
Being the report and inspection was completed very recently, in the mast 30 days, there is no further information pertaining to how the deficiencies will be addressed by the facility.
-Were there any penalties involved?
As of this time, there is no further information on any penalties which can/might be imposed on the facility for the violations noted

I decided to choose Maryland
B. From that state, identify a hospital/ health system that had project review by CON.
Anne Arundel Medical Center completed a CON application for a Cardiac Surgery Program.
C. Describe the project and the outcome of the CON process.
The CON review process is currently under review and AAMC has pledged to keep stakeholder updated during the process. (AAMC, 2016)

A. Joint Commission on the Accreditation of Healthcare Organizations (JCAHO)....define their mission/role
The Joint Commission is a not-for-profit group, operating independently, in the US that completes accreditation programs for hospitals and healthcare organizations. The JCAHO develops the standards by which crucial elements of operation are measured: patient care, medication regulation and safety, infection control and patient rights.
The laws that govern JCAHO programs are the same by which the states follow to ensure that medical costs are controlled. In connection to the ACA, the JCAHO and CON Programs have a different philosophies but similar goals with the goal of providing affordable and quality care. The JCAHO has a goal of improving on existing safety processes, whereas CONs have a goal to limit market dominance and reduce health care costs.
B. Identify and describe one accreditation standard for hospitals.
One major accreditation for hospitals is the Joint Commission accreditation. The Joint Commission is well known for demonstrating services of a high quality and is required in some environments for participation by contractor groups. The accreditation represents a “Gold Seal of Approval”(Accrediation Guideliens for Hospitals, 2016)
C. Identify a hospital by name/location that has JCAHO Accreditation. From your research, describe the published results of their last survey
Anne Arundel Medical Center is surveyed every three years by the JCAHO for maintain accreditation. The last on-site survey for accreditation was completed on 11/22/2013, resulting in the Nationwide rating being received (The Joint Commission, n.d.)

Altoona Health & Rehab. 6532 Walnut Grove Road Altoona, Alabama 35952.
-What was the deficiency?
Failed to: Store, cook, and serve food in a safe and clean way. Violating 483.70-Physical Environment ( Environmental conditions-sanitary), 483.35- Dietary Services (Nutritional Adequacy), 483.15 Quality of Life (environment-clean)
-How was the deficiency addressed by the facility?
The deficiency was inspected on 1/7/15 and was taken seriously and corrected by 2/11/2015. The facility changed the way they stored the food (storage means were enhanced-new storage holders) and cleaned up the concerned areas (kitchen and prep areas) and purchased means necessary to serve food in a safe manor (tops with lids to prevent spills).
-Were there any penalties involved?
There were no penalties listed.

The construction of a building on existing campus and the renovation of its existing 67-bed facility into a 45-bed facility. The project received a three-year extension on its expiration date. The estimated project cost is $7,904,823.

Mercy Hospital Northwest Arkansas. 2710 Rifle Medical lane Rogers, AR 72758 has JCAHO Accreditation. Last survey was done on 10/1/15 and the hospital is in good standard as of the last update.
