CANTON, Ohio - A 39-year-old worker suffered multiple broken bones after he fell more than 40 feet while conducting maintenance on a crane at TimkenSteel's Faircrest Plant. The U.S. Department of Labor's Occupational Safety and Health Administration found the company failed to provide the worker adequate fallprotection. The Aug. 7, 2015, injury was the second life-threatening injury at a TimkenSteel plant since May 2015.

OSHA cited the steel manufacturer on Jan. 29 for two repeated and three serious safety violations. TimkenSteel faces $129,500 in fines as a result of OSHA's investigation. Investigators found crane maintenance workers were exposed to falls due to lack of guardrails, and protective equipment. They also found workers were not protected from operating parts of cranes during service and maintenance because locking devices, guards and other safety procedures were not used and equipment was not powered down properly.

"TimkenSteel needs to fix their safety program immediately," said Howard Eberts, OSHA's area director in Cleveland. "OSHA will monitor these plants until the company makes protecting workers a priority. No worker should die or suffer life-altering workplace injuries because their employer fails in its responsibility to protect their employees."

In May 2015, another TimkenSteel worker was severely injured when a crane's safety latch failed and 1,000 pounds of equipment fell on him at its Gambrinus plant. In October 2015, OSHA placed TimkenSteel in the agency's Severe Violator Enforcement Program, after investigations at the Harrison and Gambrinus plants found several violations resulting in proposed fines of $393,500. The company has contested those violations.

TimkenSteel was spun off from The Timken Company in 2014. Prior to this inspection, the three plants and corporate offices now operated by TimkenSteel had been inspected by OSHA 29 times since 2005, resulting in the issuance of 76 violations. The manufacturer of large steel bars and seamless mechanical tubing was mostly recently cited by OSHA in November 2014. Its Harrison steel plant melts, rolls, produces and finishes steel, and its Gambrinus plant performs cold steel finishing.

OSHA and the Independent Electrical Contractors Inc., a trade association for electrical and systems contractors that was founded in 1957, recently renewed their alliance to protect construction workers from electrical hazards. The alliance focuses on providing agency staff with 70E and arc flash training and offers contractors guidance on preventing worker exposure to electric shock and arc flash hazards.

It's a win/win for both parties, according to John Masarick, vice president of codes and safety for the Independent Electrical Contractors Inc. (IEP).

"The reason we formed the alliance was to share information about protecting workers from electrical and arc flash hazards," said Masarick. "It's about more than OSHA regulations and enforcement."

During the five-year agreement, the alliance primarily will focus on providing OSHA staff with 70E and arc flash training, and IEC members with tools - training, videos, PowerPoint presentations, toolbox talks and more - to help them prevent worker exposures to electric shock and arc flash hazards. Through the alliance, participants also will promote awareness of OSHA campaigns on preventing falls and heat illness, as well as promote a culture of safety through outreach, particularly to small businesses and workers with limited- and non-English speaking skills.

"For more than a decade, OSHA and IEC have had a productive partnership developing resources to protect thousands of workers in the electrical industry," said Assistant Secretary of Labor for Occupational Safety and Health Dr. David Michaels. "Our continued alliance will help ensure that employers and workers in this industry receive information and training that are essential to keeping their workplaces safe and healthful."

IEC has more than 50 chapters across the country and 3,000 members, said Masarick. Many of its members are active with the NFPA 70E review panel and in helping craft updates to the National Electrical Code (NEC). Both NFPA 70E and NEC are updated more frequently than OSHA standards, and IEC - as part of the alliance - keeps OSHA inspectors up to date on changes to those standards and codes and provides them with training to spot electrical hazards.

"With chapters and members all across the country, we're in a unique position to provide training for OSHAinspectors," Masarick noted.

IEC also provides training for some 8,000 apprentices per year. They learn basic safety training, as well as training specific to electrical and arc flash hazards and fall protection. The training serves to "get the message out" about best practices in the industry and lays a solid safety foundation for the workers entering the profession, said Masarick.

Injuries Devastate Workers, Families, Employers

Many of us remember Willy Coyote cartoons showing him get shocked and afterward, looking a little singed. In reality, electrical-related injuries are no joke. They are some of the most costly in terms of pain and suffering and actual financial outlay.

Significant burns probably are the most painful injuries a worker can suffer. In addition to the initial electrical burns, particles from exploding insulation and the copper wire used in electrical wiring can imbed in the skin, causing life-threatening infections.

Serious burns often result in years of operations, painful debriding sessions, skin grafts, medical care, rehabilitation and retraining. The injuries are devastating for both the injured worker and their loved ones, who must watch their family member suffer through treatment.

In addition to the horror of knowing an employee suffered a life-altering injury, employers can face bankruptcy in the aftermath of an arc flash or electrical injury incident.

"There was one company that had an employee who was burned in an arc flash incident. The cost of the workers' compensation claim (medical and wage replacement) was $8 million. They are extremely expensive claims and a single claim can put a company out of business," said Masarick.

The best way to "deal" with electrical injuries and hazards is to prevent them, he added, and the alliance has developed fact sheets, toolbox talks and guidance documents on updates to OSHA electrical standards; hazards involved in working on or near energized electrical conductors and circuit parts; general safety guidance to help prevent fall-related injuries; and safety considerations for using ladders.

An Assist in Compliance

While some 70 percent of IEC members are small companies with fewer than 10 employees, Masarick said the association's reach is wide, encompassing more than 100,000 employees and electrician apprentices. For many of those companies and workers, IEC is a one-stop shop for training, safety management information and tools and updates about OSHA regulations, NEC changes and updates to NFPA 70E.

"Most employers want to keep workers safe, but they aren't aware of changes to regulations," said Masarick. "We keep them updated and the alliance helps us do that."

Through its alliance programs, OSHA works with unions, consulates, trade and professional organizations, faith- and community-based organizations, businesses and educational institutions to prevent workplace fatalities, injuries and illnesses. The purpose of each alliance is to develop compliance assistance tools and resources and to educate workers and employers about their rights and responsibilities. The IEC alliance program participants do not receive exemptions from OSHA inspections or any other enforcement benefits. Specifically, the OSHA/IEC alliance program:

Raises awareness of OSHA's rulemaking and enforcement initiatives.

Shares information on OSHA's National Emphasis Programs, regulatory agenda and opportunities to participate in the rulemaking process.

Disseminates information on occupational safety and health laws and standards, including the rights and responsibilities of workers and employers.

Provides outreach and communication.

Promotes awareness of OSHA's fall prevention and heat campaigns, as well as sharing of information on the recognition and prevention of workplace hazards, such as electrical shock and arc flash.

Expands existing relationships between the IEC and OSHA's regional and area offices to address health and safety issues, including the training of agency staff and providing technical expertise when appropriate.

Works with other alliance participants on specific issues and projects such as construction safety, temporary worker protection and any other issues that are addressed and developed through the alliance program (e.g., participation in the OSHA Alliance Program Construction Roundtable).

Develops materials and effective training aids that align with current regulatory information, interpretations and enforcement initiatives, as well as promotes an understanding of workers' rights and employerresponsibilities outlined in the OSH Act.

Provides for the delivery of at least four best practice training seminars annually to OSHA staff focusing on topics such as NFPA 70E and the prevention of arc flash.

Revises existing and develops new best practices for workplace safety and health management systems and other safety performance programs, as well as promotes their adoption by independent electrical contractors.

While the alliance is successful on many levels, Masarick said that one thing in particular has been beneficial for IEC members: OSHA's Consultation Program.

"A majority of companies are afraid to call OSHA for consultation help. They are afraid they'll get cited, regardless of what OSHA says," admitted Masarick. The alliance has helped them see OSHA as more of a partner than an adversary, he said, adding that more IEC members are taking advantage of the consultation program.

"We had one member contact us specifically to tell us that the OSHA Consultation Program was very beneficial to him," he said. "Another employer told us that an employee was involved in an arc flash incident but because of his training and personal protective equipment, he went home that night to his family. That's success. The alliance helps us - IEC members and OSHA - keep more workers safe."

This adverse medical phenomenon associated with being suspended in asafety harness is well-known and documented. The condition is known as suspension trauma (or harness induced pathology) and is addressed by the Occupational Safety & Health Administration (OSHA) of the U.S. Department of Labor. Design Plastering Inc., Design Plastering West LLC cited for multiple violations

DALLAS -- A fall from a third-story balcony killed 44-year-old Jorge Carrion Torres as he worked on the exterior of an apartment complex on May 14, 2015. Torres, who had been on the job for one month, was applying stucco underlayment to the balcony walls when the incident occurred. His employer had not installed scaffolding and had not provided Torres or his co-workers with personal fall protection.

The U.S. Department of Labor's Occupational Safety and Health Administration cited his employers, Design Plastering Inc. and Design Plastering West LLC, of Phoenix, on Nov. 10 for eight egregious willful and four serious violations. Previously, the state OSHA in Arizona had cited Design Plastering seven times for allowing fall-related hazards.

Federal safety and health officials have proposed fines totaling $407,400 for the citations.

"When an employer fails to put up a guardrail or scaffolding, or doesn't provide personal fall-arrest systems, anyone working at a height of six feet or more is defenseless against a fall. OSHA will not tolerate this kind of employer behavior," said Assistant Secretary of Labor for Occupational Safety and Health Dr. David Michaels.

"This senseless loss of a man's life is the result of this employer's failure to comply with clear OSHA safety requirements despite the fact that it had been previously cited for the same violations," Michaels added. "Design Plastering has been cited for $5,850 in the past for fall hazard related violations. Clearly, these penalties were not sufficient to deter this employer's unlawful actions."

The Bureau of Labor Statistics reports that one in five private industry worker deaths in 2014 were in construction. Falls are the leading cause of death for construction workers, accounting for nearly 40 percent of 2014's construction fatalities. Texas leads the nation in construction fatalities. This is the second egregious case involving fall protection there in recent months. Earlier this year, a construction worker in Houston denied a safety harness suffered severe injuries after falling through a roof.

Based in Phoenix, plaster and stucco contractors Design Plastering and Design Plastering West employ about 90 workers. They have 15 business days from receipt of the citations and penalties to comply, request an informal conference with OSHA's area director, or contest the findings before the independent Occupational Safety and Health Review Commission.

At the time of the fatal incident, Argonaut Insurance Co. was the workers' compensation carrier for the employer.

RIYADH, Saudi Arabia--A crane accident at the Grand Mosque in the holy city of Mecca killed at least 107 people, Saudi officials said, days before the start of the annual Hajj pilgrimage that will see more than two million religious tourists visit the country.

Government officials said a storm that rocked the city on Friday caused the crane's collapse, which caved in parts of the mosque--the world's largest--filled with pilgrims.

Footage aired by state television showed police and medical teams attending to bloodied bodies on the mosque's white marble floors, as high wind and heavy rain lashed the site.

An additional 238 people were reported injured, according to the official Twitter account for the nation's Civil Defense department.

A spokesman for the health department in Mecca region told the state news agency it was still working to identify the nationalities of the victims.

Prince Khaled al-Faisal, the governor of Mecca, ordered an urgent investigation into the accident, Saudi state television said. He later visited the accident site and ordered officials to begin repairs as soon as possible.

The Grand Mosque pulls Muslims from around the world for the Hajj. Millions, dressed in simple white clothes, perform prayer rituals for what is often a once-in-a-lifetime religious pilgrimage.

Yet the massive crowds have contributed to several tragedies at Islam's holiest of sites, most of them due to stampedes.

The last deadly incident occurred in 2006, when 346 were killed in a stampede on Mecca bridge. In 1990, a stampede in a pedestrian tunnel in the city killed 1,426 pilgrims, the deadliest Hajj incident in recent history.

Abdur Rahman Laily, a 33-year-old pilgrim from South Africa, said he heard the strong impact of the crane falling from his hotel room near the Grand Mosque. He soon headed to the mosque to see.

"This was one of the tallest and biggest cranes at the mosque expansion project," he said in a telephone interview from Mecca.

Mr. Laily said the falling crane made a large hole at one of the upper levels used by pilgrims to walk around the Kaaba, a stone cube covered in black at the center of the mosque and known as the House of God.

Workers quickly cleaned the debris but that level was no longer accessible, he said.

The accident has increased pressure on the Saudi authorities at a time they face a series of economic and security challenges.

The oil-rich country is dealing with a sharp drop in the price of crude, which is now at its lowest level since March 2009.

Meanwhile, as it jockeys with Iran for regional influence, it has been spearheading fighting in neighboring Yemen against Houthi rebels, who are accused by the Saudis of receiving support from Iran. The fighting, which aims to reinstall Western-backed president Abed Rabbo Mansour Hadi, has dragged on for more than five months with an estimated cost of more than $1 billion.

Protecting the pilgrims and ensuring their safety is an especially sensitive issue for a country that prides itself as a leader of the Islamic world and where the king's official title is "Custodian of the Two Holy Mosques."

In recent years, Saudi Arabia has limited the number of people allowed in for the pilgrimage to accommodate construction for the city's expansion projects. The construction at the mosque, set to finish next year, was slated to increase the mosque's capacity significantly.

Already, more than half a million pilgrims have arrived in the kingdom in preparation for Hajj this year, even as construction continued at the mosque and surrounding areas.

Saudi Binladin Group, the commercial vehicle of the bin Laden family and one of the country's biggest construction firms, which has been leading the expansion project, had suspended the project on Thursday, according to local daily Saudi Gazette.

The company didn't immediately respond to requests to comment.

It wasn't immediately clear who owned the crane.

Saudi officials have warned pilgrims to avoid any actions that might disrupt Hajj.

"Any act contrary to the regulations and instructions while performing the rituals" of pilgrimage "will be treated with the utmost firmness," the state news agency said last month.

But with waves of people who speak different languages packing into a confined area, crowd control has become a serious challenge.

Saudi authorities also are attempting to manage concerns about the spread of the lethal Middle East respiratory syndrome, or MERS. The country reported 34 new cases of the disease last week.

The Kingdom last week announced a ban on the practice of sacrificing camels and sharing their meat with the poor during Hajj--a step taken to limit the spread of the virus, which is believed to have been passed from infected camels to humans.
